血清糖类抗原与肿瘤特异性生长因子联合检测在胃癌临床诊断中的应用

摘　　要：目的探讨血清中肿瘤相关糖类抗原72-4(CA72-4)、肿瘤相关糖类抗原19-9(CA19-9)和肿瘤特异性生长因子(TSGF)3项联合检测对胃癌临床诊断的意义。方法采用电化学发光免疫分析技术对60例胃癌,55例胃良性疾病患者,50例健康对照血清进行CA72-4、CA19-9、TSGF3项检测,并用各单项及组合对胃癌的诊断的敏感性、特异性、阳性预测值和阴性预测值进行比较,确定其联合检测对胃癌诊断的效能。结果胃癌组的血清中CA72-4、CA19-9、TSGF的水平明显高于胃良性疾病组和健康对照组,3种标志物诊断胃癌的敏感性分别为62.4%、68.0%、54.4%,特异性分别为95.7%、86.6%、90.2%。3项联合检测敏感性提高到86.8%,与单项检测比较差异有统计学意义(P<0.05);同时特异性也具有较高水平,达95.3%。结论CA72-4、CA19-9、TSGF可作为临床诊断胃癌的肿瘤标志物,3项联合检测可明显提高阳性检出率。Objective To investigate the clinical value of CA72-4, CA19-9 and TSGF in serum of the patients with gastric cancer and its significance in diagnosis. Methods Serum Levels of CA72-4, CA19-9 and TSGF was detected by using electro chemiluminesence immunoassay （CEL）and serum levels of TSGF was detected by using ELISA in 60 cases of patients with gastric cancer , 55 cases of patients with gastric benign disease and 50 normal health persons . Diagnostic values of these tumor markers were analyzed by sensitivities ,specifieities, sensitivities positive prospective value and negative prospective value respectively. Results levels of CA72-4,CA19-9 and TSGF in the serum of gastric cancer group were significantly higher than those in gastric benign disease and normal health group（P〈0. 01）. and the sensitivities were 42.8%, 45. 7% and 54. 5% respectively, and the speeificities were 90.7% ,86.6% and 87.4%, respectively in gastric cancer. The sensitivity, and sensitivities and specificities of combined detection of three tumor markers were 86.8%, and 95.3% respectively. Conclusion CA72-4,CA19-9 and TSGF can serve as the serum tumor marker in clinical diagnosis of gastric cancer, Combined detection of serum tumor makers show higher clinical values in gastric cancer diagnosis.
