摘 要:目的观察银杏叶提取物(GBE)对大鼠颅脑损伤后心肌病变的保护作用及神经肽Y(NPY)基因表达的影响。方法采用液压冲击建立颅脑损伤模型和药物干预模型,行心导管插管测定左心室收缩压(LVSP),左心室舒张压末压(LVEDP)及左心室压力上升和下降最大速度(±dp/dtmax),同时测定血清肌酸磷酸激酶同工酶(CK-MB)含量的改变,并通过反转录-聚合酶链反应(RT-PCR)的方法研究大鼠颅脑损伤后心肌NPY基因的表达。结果与对照组大鼠比较,颅脑损伤组大鼠LVSP和±dp/dtmax明显下降(P<0.05),LVEDP明显升高(P<0.05),血清CK-MB含量明显升高(P<0.05),且心肌NPY基因的表达水平显著增加(P<0.05);与颅脑损伤组大鼠比较,GBE干预组大鼠LVSP和±dp/dtmax增加(P<0.05),LVEDP降低(P<0.05),血清CK-MB含量降低(P<0.05),且心肌NPY基因的表达水平减少(P<0.05)。心肌NPY基因的表达与血清CK-MB和LVEDP变化呈正相关(r值分别为0.988和0.957,P<0.05),与LVSP变化呈负相关(r=-0.960,P<0.05)。结论颅脑损伤可引起明显的心肌病变,GBE对大鼠颅脑损伤后心肌病变有保护作用,其作用可能与心肌NPY基因表达下调有关。Objective To explore the protective effects of Ginkgo Biloba extract (GBE) on myocardial damage after traumatic brain injury in rats and on the expression of neuropeptide Y (NPY) mRNA in myocardium. Methods Fluid percussion induced TBI models and GBE-pretreated models were established in rats. Left ventrieular systolic pressure (LVSP), left ventricular end diastolic pressure (LVEDP) and + dp/dtmax were measured by multipurpose polygraph under left cardiac catheterization. The level of MB isoenzyme of creatine kinase (CK-MB) in serum was also determined. The expression of NPY in myocardium was detected by semi-quantitative reverse transcription polymerase chain reaction (RT-PCR). Results Compared with control group, LVSP. and +dp/dtmax were lowered (P〈0.05), whereas LVEDP, level of CK-MB and expression of NPY in myoeardium were significantly higher in the TBI group (all P〈 0.05). Compared with TBI group, GBE-pretreated group showed higher levels of LVSP and +dp/dtmax (both P〈0.05), lowered LVEDP, level of CK-MB and expression of NPY in myocardium (all P〈0.05). Correlation analysis showed that the expression of NPY in myocarclium was positively correlated with the level of CK-MB (r=0.988,P〈0.05) and LVEDP (r=-0.957, P〈0.05), and the expression of NPY in myocardium was negatively correlated with LVSP (r=-0.960, P〈0.05). Conclusi6n Traumatic brain injury can result in myocardial damage, and GBE can protect myocardium by down-regulating the expression of NPY in myocardium.
