摘　　要：目的了解某三级甲等医院三种清洁手术围手术期抗菌药物预防性使用的合理性。方法对2008年9月份38例清洁手术围手术期抗菌药物使用情况进行回顾性调查分析。结果三种清洁手术围手术期抗菌药物使用率为86.84%。使用频率最高为头孢菌素类药物。66.67%的药物DUI<1。抗菌药物使用不合理占68.42%。使用不合理现象主要表现为药物种类选择不合理、疗程过长、换药过频、给药方法不当等。结论某三级甲等医院三种清洁手术围手术期预防性抗菌药物使用普遍存在不合理现象,需建立有效的监管机制以促进药物应用的合理化。Aim To get the messages of rationality about prophylactic use of antibacterials in perioperative period about 3 kinds of clean surgery in a third-grade elass-A hospital. Methods A retrospective study was made of the prophylactic use of antibacterials in perioperative period about 38 cases of clean surgery in September 2008. Results 86.84% of the clean sugery was found to have prophylactic use of antibaeterials in perioperative period and cephalosporins were used most frequently. 66.67% of drugs showed DUI no more than 1. 68.42% of use of the antibacterials was not rational. They are manifested as improper choise, prolonged use or frequent drug change etc. Conclusions There are unrational prophylactic use in pefioperative period about 3 kinds of clean surgery in a third-grade class-A hospital. Rational use should be emphasized by effective supervision and management.
