摘 要:目的:观察养血清脑颗粒对急性脑梗死患者的临床疗效及血液流变学和血浆内皮素(ET-1)的作用。方法:将255例急性脑梗死患者分为常规治疗(对照)组120例和养血清脑颗粒(治疗)组135例。对照组静脉滴注甘露醇、胞二磷胆碱、维脑路通及口服脑复康、阿司匹林;治疗组在对照组治疗基础上,从首日加服养血清脑颗粒(每袋4g,每次1袋,每日3次),连用8周。观察2组的临床疗效及血液流变学和ET-1水平的变化。结果:治疗组总有效率为88%,对照组为93%,2组间比较无显著性差异,但治疗组总显效率(64%)显著高于对照组(30%)(P<0.01)。2组患者治疗前、后血小板计数无明显变化;凝血因子-Ⅰ在用药后均明显下降,但2组间比较无显著性差异;2组治疗8周后,血细胞比容、全血黏度和血浆黏度均有明显改善,但组间比较无显著性差异;治疗组血浆ET-1水平下降(P<0.01),与对照组比较有显著性差异(P<0.01)。结论:治疗组对急性脑梗死患者治疗后的总显效率明显高于对照组,可能与养血清脑颗粒能降低血细胞比容和全血黏度有关,调控血管活性物质可能是其关键。OBJECTIVE: To evaluate the curative efficacy of Yangxue qingnao granule and its effects on the hemorrheology and endotheline(ET- 1) in patients with acute cerebral infarction(ACI) .METHODS: A total of 255 patients with ACI were divided into two groups: 120 in the control group received routine therapy (mannitol, citicoline, troxerutin, piracetam, and aspirin), and the other 135(trial group) received the above - mentioned routine therapy in combination with Yangxue qingnao granule(4 g/pouch, one pouch/time, t.i.d) for 8 weeks. The clinical efficacy in two groups were observed with the scores of nervous system defect as outcome measures, meanwhile, the changes of hemorrheology and ET- 1 after treatment were observed as well. RESULTS: There were no significant differences between the trial group and the control group in total effective rate(88% vs. 93%),but significant differences were noted in total remarkable- efficacy rate (64% vs. 30%, P〈 0.01). The changes in blood platelets count for both groups were nonsignificant before and after treatment. Blood coagulation factor- I levels in both groups were markedly decreased at 4 and 8 weeks, but no significant differences were noted between the two groups. At 8 weeks, the haematocrit, blood viscosity and plasma viscosity in both groups improved significantly and there were no significant differences between the two groups. Plasma ET- 1 in the trial group decreased significantly after treatment (P( 0.01) and the differences between the two groups were significant(P 〈0.01).CONCLUSION: The higher total remarkable- efficacy rate noted in the trial group is associated with the action of Yangxue qingnao granules in down - regulating haematocrit and blood viscosity, and its regulatory effect on vasoactive substance may play a key role.
