广西壮族腰椎间盘突出症血瘀证患者与TGF-β1-509C/T基因多态性及部分危险因素的关联研究

摘　　要：[目的]探讨腰椎间盘突出症血瘀证患者及部分危险因素与TGF-β1-509C/T基因多态性的关联性。[方法]筛选符合既定相关标准的广西壮族人群腰椎间盘突出症患者88例,分为血瘀证组30例与非血瘀证组58例,进行TGF-β1-509C/T基因多态性检测,并观测患者的腰椎负荷强度等指标,观察部分危险因素以及部分危险因素和TGF-β1-509C/T基因多态性的交互作用与腰椎间盘突出症中医证型的关联性。[结果]经非条件Logistic回归For-ward(LR)法分析显示广西壮族重度腰椎负荷强度者患腰椎间盘突出症血瘀证的风险是轻中度腰椎负荷强度者的3.911倍(95%CI:1.334～1.872)(P<0.05),TGF-β1基因-509C/T多态性与重度腰椎负荷强度的累积暴露发生腰椎间盘突出症血瘀证的风险是其中某单一因素的18.425倍(95%CI:1.767～3.693)(P<0.01)。[结论]重度腰椎负荷强度很可能是广西壮族人腰椎间盘突出症血瘀证的潜在危险因素之一,TGF-β1基因-509位点CT基因型与重度腰椎负荷强度累积暴露更易发生血瘀型腰椎间盘突出症。[Objective] To investigate the relationship between the blood stasis syndrome and parts of the risk factors of lumbar intervertebral disk protrusion（LIDP）and transforming growth factor β1（TGF-β1）-509C/T site gene polymorphisms.[Methods]A total of 88 Zhuang ethnic patients from Guangxi Zhuang Autonomous Region were selected according to standard of LIDP,30 with blood stasis syndrome and the other 58 without,then,their single nucleotide polymorphism（SNP）of TGF-β1-509C/T and risk factors like lumbar burden intensity were tested,and their relationship with the traditional Chinese medicine syndrome of LIDP were analyzed.[Results]It was showed by logistic regression Forward（LR）analysis that risk of syndrome of blood stasis pattern of LIDP in patients with severe lumbar burden was 3.911 times（95%CI：1.334～1.872）of that in patients with mild and middle lumbar burden（P〈0.05）,and the risk further increased 18.425 times（95%CI：1.767～ 3.693）after it＇s accumulating with TGF-β1-509C/T site gene polymorphisms in Zhuang ethinic group in Guangxi（P〈0.01）.[Conclusion]Severe lumbar burden may be one of the risk factors of syndrome of blood stasis pattern of LIDP in Zhuang ethnic populational from Guangxi.Patients with TGF-β1 gene-509 adiponectin CT genotype and combined severe lumbar burden are susceptible to syndrome of blood stasis pattern of LIDP.
