原发性高血压病患者血尿酸水平与颈动脉粥样硬化的关系

摘　　要：目的探讨原发性高血压病患者血尿酸水平与颈动脉粥样硬化的关系及机制。方法61例原发性高血压病患者分为Ⅰ级、Ⅱ级、Ⅲ级三个亚组,同期非高血压病患者53例。对所有患者行血尿酸值测定和颈动脉超声检查,并对各组数据行统计分析。结果血尿酸均值高血压病组为(339.3±121.3)μmmol/L,明显高于对照组的(295.1±89.8)μmmol/L(P<0.05);高血压病Ⅲ级患者血尿酸水平明显高于Ⅰ级和Ⅱ级者(P<0.01)。超声颈动脉粥样硬化检出率高血压病组为52.46%,显著高于对照组的35.85%(P<0.05);高尿酸血症(HUA)检出率高血压病组为29.51%,明显高于对照组的11.32%(P<0.05);颈动脉粥样硬化与HUA检出率不同分级高血压病组间差异无统计学意义(P>0.05)。高血压病并发HUA患者超声检出颈动脉硬化率升高至61.11%,高于未并发HUA者的48.8%(P>0.05)和对照组单纯HUA的11.32%(P<0.05)。结论高血压病患者血尿酸水平升高,容易发生HUA和颈动脉粥样硬化疾病,高血压病合并HUA发生颈动脉粥样硬化风险增高。Objective To analyze the correlation and mechanism between the level of serum uric acid and carotid artery atherosclerosis in patients with essential hypertension（EH）.Methods A total of 61 patients with EH（divided into gradeⅠ,Ⅱ and Ⅲ）and 53 patients without hypertension were enrolled.The value of serum uric acid was measured and carotid ultrasound was performed.Results The mean value of serum uric acid in patients with or without EH was（339.3±121.3）μmmol/L and（295.1±89.8）μmmol/L,respectively（P〈0.05）,and hypertension grade Ⅲ was significantly higher than that of grade Ⅰ and Ⅱ（P〈0.01）.The detection rate of carotid atherosclerosis with ultrasound in cases with EH（52.46%）was higher than that in non-hypertension group（35.85%,P〈0.05）.The rates of hyperuricemia and carotid atherosclerosis were not had statistical different among grade Ⅰ,Ⅱ and Ⅲ（P〉0.05）.The rate of carotid atherosclerosis in group hypertension with hyperuricemia was higher than that either in group hypertension without HUA（P〉0.05）or in group non-hypertension with hyperuricemia（P〈0.05）.Conclusion The patients with EH have high level of serum uric acid and will easily suffer hyperuricemia and carotid atherosclerosis.Hypertension with hyperuricemia increases the risk of carotid atherosclerosis.
