摘  要:目的分析前路手术在胸腰椎骨折治疗中的手术方法和临床治疗效果,探讨其应用价值。方法于1997—2006年对108例胸腰椎骨折患者行前路手术内固定治疗,评估胸腰椎前路固定术后脊柱畸形矫正度、患者神经功能康复状况及术后并发症发生情况。结果术后随访1~7年,平均4.8年,无一例因并发症死亡,畸形矫正满意,脊柱高度恢复,内固定系统无松动、断裂,脊髓神经功能有不同程度的恢复,无松动、断钉等并发症发生。患者术后及随访时的后凸角、侧凸角与术前比较,差异均有统计学意义(P<0.05);患者术前、术后的神经功能状况比较,差异有统计学意义(P<0.05)。结论胸腰椎骨折采用前路手术治疗安全可靠,对脊髓减压充分彻底,可矫正畸形,重建脊柱的稳定性,促进神经功能恢复。Objective To analyze the surgical technique by anterior approach for thoracolumbar spinal fracture and its clinical outcome so as to explore its value of application. Methods A total of 108 patients with thoraeolumbar fractures were operated with anterior decompression and fixation technique (ADFT) from 1997 to 2006. And the correction degrees of spinal deformity, neurological rehabilitations condition and post-operative complications of the patients after anterior thoracolumbar fixation were assessed. Results The duration of follow up ranged from 1 to 7 years with an average of 4.8 years, and no case of death occurred because of complications. Satisfactory correction of deformity was noticed, the height of spinal column recovered, no loosing or split of the inner fixation system was found, the neurological function recovered to a certain degree, no complication was found. The angles of kyphosis and scoliosis were reduced after the operation (P〈0.05). The postoperative recovery of neurological function was of significance statistically (P〈0.05). Conclusion The anterior decompression and fixation technique is safe for thoracolumbar spinal fracture The decompression of cord is complete, the spinal stability is rebuilt and the neurological function recovery is improved.
