摘  要:目的探讨人工全髋关节置换术后下肢延长对髋关节功能的影响。方法初次THA患者143例,摄标准骨盆平片,测量患肢和对侧肢体长度,取得患肢的延长值,并对患髋进行Harris髋关节功能评分。根据下肢长度的差值(LLD),将患者分成四组:等长组(LLD≤5 mm),A组(5 mm<LLD≤10 mm),B组(10 mm<LLD≤20 mm),C组(LLD>20 mm),分析各组间术后Harris髋关节功能评分和患者满意率。结果人工全髋关节置换术后等长组39例(27.2%),A组77例(53.8%),B组20例(14.0%),C组7例(4.9%)。Harris髋关节功能评分:等长组高于B、C组,B组高于C组(P<0.05)。等长组、A组、B组和C组的满意率分别为94.9%、88.3%、60.0%和14.2%。结论人工全髋关节置换术术后下肢延长是较普遍的现象,患肢延长超过1 cm对髋关节功能有一定影响。Objective To study the influence of leg lengthening after total hip arthroplasty (THA) on hip function. Methods One hundred and forty-three cases underwent THA were examined with standard radiograghs of the pelvis for determining the leg length discrepancy(LLD) by measuring the lengths of affacted and contralateral limbs. Harris hip score(HHS) was recorded. According to LLD ,the cases were divided into four groups of LLD≤5 mm(group N,equal length),5 mm〈LLD≤10 mm (group A) ,10 mm〈LLD≤20 mm (group B) ,and LLD〉20 mm(group C). The HHS scores and satisfaction rates were compared among four groups. Results Of 143 cases,39 cases (27.2%) entered into group N,77 cases(53.8%)into group A,20 cases(14.0%)into group B, and 7 cases(4. 9%)into group C. HHS score was higher in group N than that in group B or group C,which was higher in group B than that in group C (P〈0. 05). The satisfaction rates of groups of N,A,B and Cwere 94.9%,88.3%,60.0%,and 14.2%,repectively (P〈0.05 or P〈0.01). Conclusion Leg lengthening after THA is common. The hip function is impaired when the LLD is more than 1 centimeter.
