摘　　要：目的探讨使用HoffmannⅡ外固定支架治疗复杂性胫腓骨骨折的常见并发症及其防治。方法我们选择HoffmannⅡ外固定支架治疗复杂性胫腓骨骨折35例,采用C臂X线机透视下间接复位或小切口直视下复位,根据术中骨折局部稳定情况,设计外固定架构型并固定骨折。分析术后产生各种并发症的原因并提出防治措施。结果术后平均随访10个月。骨正常愈合24例,骨延迟愈合9例,未愈合2例,针道感染7例,针道无菌性炎症反应8例,固定钢针松动11例。结论外固定支架对治疗某些特殊类型的胫腓骨骨折骨折有一定优势,但其并发症较多,应严格掌握使用指征。针道感染,固定钢针松动,骨折延迟愈合与骨不连是其主要并发症。正确操作,针孔干燥、引流通畅是防治并发症的有效措施。Objective To study the common complications and prevention of complex tibia and fibula fracture treated by Hoffmann Ⅱ external fixator. Methods We chose Hoffmann Ⅱ external fixator to treat tibia and fibula fracture in 35 cases, analyzed the complications and carded out the prevention measures. Results After operations, the mean follow-up is 10 months, ranging from 7 months to 15 months. The bones were united normally in 24 cases, delayed united in 9 cases, nonnnited in 2 cases; and 7 cases were found with the pin tract infects, 8 cases with the pin tract aseptic inflammation responds, Ⅱ cases with the pin loosening. Conclusion The external fixator used to treat certain special types tibia and fibula fracture was proved to have certain superiority. However, with so many com- plications, we should grasp the correct usage for indications. The main complications includes pin tract infection, the pin loosening, the bone delayed union and nonunion. The proper operation, keeping the pin tract xeransis, the drainage unobstructed are effective measures to prevent and control the complications.
