摘 要:目的观测中药膏剂"CYG"对大面积全厚皮肤缺损创面的促愈效果并探讨其促愈机制。方法贵州小型猪7只,每只背部脊柱两侧近脊柱均有全厚皮肤缺损创面(大小10.5cm×8.5cm)各2个,共28个创面。右侧的14个创面用中药膏剂"CYG"治疗作实验组,同体左侧创面作对照。观察伤后7周内各创面愈合时间、愈合率及愈合过程病理变化,并通过S-P法层粘蛋白(Laminin)免疫组化染色观测其肉芽组织内血管生成变化,免疫组化染色、RT-PCR法检测转化生长因子-β1(transforming growth factor beta one,TGF-β1)表达。结果伤后7~44d,治疗组与对照组创面平均残留面积及愈合残留面积百分率比较,差异有统计学意义(P<0.01),治疗组于伤后32~34d完全愈合,较对照组提前10~12d,且创面质地细腻、平整。愈合过程中治疗组肉芽组织生长早且旺盛,肉芽组织中Laminin、成纤维细胞、毛细血管及胶原含量丰富;治疗组各时相点创面TGF-β1表达较对照组低,同组TGF-β1表达:中期(伤后15~35d)>早期(伤后7~14d)>晚期(伤后66~49d)。结论中药膏剂"CYG"对大面积全厚皮肤缺损创面有良好促愈作用,且创面愈合质量高,并可能对创面瘢痕形成具抑制作用。Objective To investigate the effectiveness and mechanism of traditional Chinese ointment "CYG" on the promoting of wound healing of large-area full-thickness cutaneous defect. Methods Seven minipigs were employed in this study. Two full-thickness cutaneous wounds at a size of 10.5 cm×8.5 cm were produced on the dorsum area of each side close to the vertebral column in each animal. Among 28 wounds, 14 right side wounds were treated with traditional Chinese ointment "CYG" as experimental group, and 14 left side wounds without treating as control. Subsequently the healing time, healing rate and healing pathological change of each wound surface during the 7 weeks' healing procedure were observed, and immunohistochemistry staining was carried out with antibody against laminin to test the angiogenesis in the granulation tissue of the same wound surface. Immunohistochemistry staining and reverse transcriptase polymerase chain reaction (RT-PCR) were employed to examine the expression of transforming growth factor beta 1 ( TGF-131 ) of the same granulation tissue. Results In 7 to 44 d after injury, there were significant differences in new area of wound and healing rate between 2 groups (P 〈0. 01 ). The healing time of experimental group was 32 to 34 d, and 10 to 12 d faster than that of control group. The wound surface of experimental groups grew with exquisite and planus tex- tures and their granulation tissue, which contained a lot of laminin, fibroblasts, capillaries and collagen, and grew earlier than that of control groups. Expression of TGF-[M at different interval time in experimental group was lower than that of control group, and this expression in wounds of the same group were ranked in the follow- ing order: intermediate stage ( 15 to 35 d postinjury) 〉 early phase(7 to 14 d postinjury) 〉 late stage (36 to 49 d postinjury) throughout healing procedure.
