摘 要:目的建立一种快速冷冻红细胞制备的新方法,为急诊、野战等特殊要求下提供血液保障。方法以甘油为保护剂,-80℃保存红细胞,使用前采用微波水热法解冻,用不同浓度的洗涤液洗涤去除甘油,以红细胞回收率、游离血红蛋白含量、残余甘油含量、体外溶血试验和无菌试验为质量检测控制项目。结果储存7天内的红细胞悬液,制成浓缩红细胞,10ml/min、150次/分水平摇摆条件下分2次加入57.1%的复方甘油溶液,中间水平静置平衡10min,使甘油浓度达到40%(w/v),-80℃保存。需要时,采用微波水热法解冻,20ml/min、150次/分水平摇摆条件下,分别用9%NaCl溶液、羟乙基淀粉40氯化钠溶液、0.9%NaCl溶液去除甘油,再用0.9%NaCl溶液制成冷冻红细胞悬液。20袋制品质量指标好于原传统方法,均符合国家标准,l临床使用效果良好。结论新建的冷冻红细胞制备方法,优于传统方法,而且制备时间缩短了45min,能快速地为临床提供所需血液,特别在野战环境、抢险救灾等特殊要求下能提供有力保障。Objective To establish a new method for rapid making the frozen red blood cells according to the especial demands of blood service emergency and in the field environments. Methods We used glycerine as protective fluid, kept RBC in the refrigerator at -80℃,unfreezed it with microwave - hydrothermal method, and removed the glycerine with several different washing solutions in concen- tration. The rate of recovery of RBC, the content of free Hb, the content of the glycerine residue, the test of external hemolysis and the a-septic test were taken as the projects of quality control. Results The red ceils suspension stored in 7 days was added into 57.1% glycerine solution for 2 times in 10ml/min and 150 bpm zero - level swing condition, during keeping untouched for 10 mins, to make the glycerine concentration up to 40% (w/v) , which was kept in the refrigerator at -80℃. When required, it was unfreezed with microwave -hy- drothermal method and in 20 ml/min and 150 bpm zero -level swing condition, glycerine was removed using 9% NaC1 solution, hydroxyethyl starch 40 sodium chloride solution, and 0.9% NaCl solution, and then it was mix -suspended with 0.9% NaCl solution. All the quality-indexes of 20 bags of production prior to the traditional methods, which comply with to the state demands, also aeguired a good clinical efficiency. Conclusion The new method is better than the traditional methods. It can decrease preparing time for 45 min and can fast supply the red blood cells ,especially according to the especial demands of blood service in the field environments and the disaster e-mergency.
