摘  要:目的:检测系统性红斑狼疮(SLE)患者血浆纤维蛋白原(FIB)水平,分析其与SLE疾病活动及脏器损害的关系。方法:测定68例SLE患者与36名正常对照血浆中的FIB浓度,分析其与SLE病情活动及其他临床指标的相关性。结果:SLE病例组和健康对照组相比,血浆FIB水平显著升高,差异有统计学意义(P<0.01)。不同疾病活动度的SLE患者FIB水平差异有统计学意义(P<0.01)。血浆FIB水平与SLE患者病程中出现的肾脏、心血管等脏器损伤存在一定联系。FIB与SLE患者的疾病活动指数(SLEDAI)、24h尿蛋白定量、免疫球蛋白G(IgG)、红细胞沉降率(ESR)等指标成正相关。结论:FIB可能参与了SLE的疾病活动及脏器损伤,临床上可作为疾病活动及脏器损伤的观察指标。Objective: To detect the plasma level of fibrinogen (FIB) in systemic lupus erythematosus (SLE) patients, and analyze the correlation with disease activity as well as organ damage. Methods: The plasma level of FIB was measured by immunoenhancement in 68 SLE patients and 36 healthy controls. At the same time , the patients's SLE disease activity index SLEDAI, laboratory data were also collected. Results:The plasma level of FIB was significantly higher in SLE patients than in healthy control group (P〈 0.01). The plasma level of FIB was higher in active patients than in inactive patients. Patients with cardiovascular and cerebrovascular disease, mucocutaneous damage ,renal damage, lung interstitial disease , their plasma level of FIB were higher than that of patients with non-organ involvement (P〈0.01). The plasma level of FIB was higher in ACL -positive group than in ACL-negative group (P〈0.05). Plasma FIB concentration correlated positively with SLEDAI score (P〈0.01), 24-hour urine protein excretion (P〈0.01), serum IgG(P〈0.01)and ESR(P〈0.01), but negatively correlated with serum C3 level (P〈0.01).There was no correlation between the plasma level of FIB and serum IgA, IgM level. Conclusion: FIB may be involved in the activity and organ damage of SLE. It may serve as an active disease marker of SLE
