摘  要:目的:建立人血清中去甲万古霉素浓度的HPLC测定方法及临床应用。方法:色谱柱为Waters C18柱(150mm×4.6mm,5μm);流动相为磷酸二氢钾缓冲液-甲醇-乙腈(86:10:4);流速1mL.min-1;检测波长236nm;以万古霉素为内标,采用硫酸锌直接沉淀蛋白后进样。结果:去甲万古霉素血清浓度与峰面积线性关系良好,Y=0.0348C-0.0418,r=0.9998;线性范围2.0~100.0mg.L-1;日内RSD小于5.6%;日间RSD小于6.3%;去甲万古霉素高、中、低浓度的平均方法回收率分别为101.36%、97.63%、99.57%;平均提取回收率分别为89.84%、88.82%、88.68%。该方法适于ICU患者接受治疗时去甲万古霉素的血药浓度监测。结论:方法操作简便快速,回收率稳定,适于临床血药浓度监测及人体药动学研究。Objective: To establish an HPLC assay for the determination of norvancomyein in human serum using vancomycin as internal standard ( IS). Methods : The pretreatment of the blood samples was simply sedimentation with 10% ZnSO4. Then the sample was separated on a Waters C18 column (150mm×4.6mm,5μm) with a mixture of potassium dihydrogen phosphate-methanol-acetonitrile (86: 10:4) as the mobile phase. The UV detecting wave length was 236 nm. The flow rate was 1.0 mL·min^-1. The temperature of the column was 35℃. Results: A linearity equation as Y=0. 034 8C -0.041 8 was obtained from norvancomycin concentration of 2 to 100 mg·L^- with a good correlation (r =0. 999 8). The values for intra-day and inter-day RSD were less than 5.6% and 6.3% , respectively. It was suitable for the therapeutic drug monitoring of norvancomycin in ICU patients. Conclusions: The propsed method appears to be a rapid, convenient and accurate method to monitor the norvancomycin concentration in serum and can be used for its pharmacokinetic study.
