食用三聚氰胺污染的婴幼儿配方奶粉的小儿泌尿系统筛查情况分析

摘　　要：目的:探讨与食用三聚氰胺污染的婴幼儿配方奶粉相关的婴幼儿泌尿系结石的临床特点、诊断和治疗措施。方法:回顾分析2008年9月～10月中日友好医院儿科门诊2539例食用可疑受三聚氰胺污染的婴幼儿配方奶粉的小儿泌尿系统筛查情况。筛查项目包括尿常规和泌尿系超声检查。对确诊因食用受污染配方奶粉致泌尿系结石的患儿行血尿素氮、肌酐检测。分析三聚氰胺相关泌尿系结石的流行病学、临床及影像学特点。结果:2539例接受筛查的小儿中结果异常者54例,其中21例患儿初步诊断三聚氰胺相关泌尿系结石。泌尿系超声检查提示双侧肾结石6例,左肾结石8例,右肾结石7例,其中2例合并单侧肾盂积水和肾盂扩张。21例肾结石患儿血尿素氮、肌酐均正常。结论:食用三聚氰胺污染的婴幼儿配方奶粉可以导致婴幼儿泌尿系结石。泌尿系超声检查对诊断、鉴别诊断三聚氰胺相关泌尿系结石十分重要。对较小结石、无尿路梗阻和临床症状者,无需特殊干预,可定期随诊;对较大结石、有短期部分尿路梗阻者多首选内科保守治疗。Objective ：To summarize clinical characteristics ,diagnosis and treatment of infants with urinary calculus developed after being fed with melamine tainted formula milk.Methods：2539 infants after being fed with melamine tainted formula milk enquiringly admitted to the China-Friendship Hospital pediatric out-patient department from 2008 September to October.The urine routine and urinary ultrasonic features,blood urea nitrogen （BUN）and creatinine （Cr）were checked to analyze the epidemiological characteristic,clinical manifestations as well as image features.Results：21 children were diagnosed as urinary calculus due to melamine tainted formula milk,6 bilateral renal calculus,8 left and 7 right renal calculus,2 patients were complicated with half pelvis hydrops or distension but their BUN and Cr were normal.Conclusion：Melamine tainted formula milk may cause urinary calculus.Urinary ultrasonic features are the pivotal method to diagnose and differentiate the infant urinary calculus due to melamine tainted formula milk.Infants have no severe adverse renal outcomes such as acute renal failure or urinary tract obstruction require follow-up and none specific treatment.
