126例糖尿病足疗效分析

摘　　要：[目的]探讨糖尿病足内外科综合治疗的临床疗效体会。[方法]自2005年1月～2008年12月间,126例糖尿病足采用胰岛素控制血糖,积极抗感染,改善微循环,合理清创换药,以及烧伤科植皮、骨科截肢或截趾等综合治疗。本文对其治疗结果进行回顾性分析。[结果]88例(69.8%)经内科治疗后痊愈;38例(30.2%)转外科植皮、截肢或截趾治疗;而其中Wagner分级3级以上者69例,内科治疗好转31例(44.9%),转外科38例(55.1%)。[结论]糖尿病足与慢性高血糖及其他多种因素有关,需胰岛素控制血糖、抗生素控制感染基础上进行综合治疗,可提高治愈率,降低致残率;加强糖尿病足教育有助减少糖尿病足的发生。[Objective] To investigate the clinical curative effect of diabetic foot treated with combined therapy of medicine and surgery.[Methods] From January 2005 to December 2008,126 cases patients with diabetic foot were treated with combined therapy such as controlling blood sugar with insulin,positive anti-infection,improving microcirculation,de-briding and changing dressings reasonably,skingrafting,amputation and toe amputation,et al.The therapeutic results were analyzed retrospectively.[Results] 88 cases patients with diabetic foot（69.8%）were cured by medicine.38 cases（30.2%）were treated with skingrafting,amputation or toe amputation.Of the people who got the treatment,the Wagner grade of 69 cas-es patients was above three.Only 31 cases patients（44.9%）were improved by medicine,and 38 cases patients（55.1%）needed to be treated by surgery.[Conclusion] Diabetic foot is associated with chronic hyperglycemia and other kinds of fac-tors.On the base of controlling blood sugar with insulin and controlling infection with antibiotics,the combined therapy is need-ed to increase cure rate and decrease mutilation rate.To strengthen the education of diabetic foot is helpful to decrease the occurrence of diabetic foot.
