影响COPD机械通气患者成功撤机的临床因素研究

摘　　要：目的:研究影响COPD机械通气患者成功撤机的临床因素。方法:将1201例COPD机械通气患者按是否成功撤机分为撤机成功组和撤机失败组,比较两组患者撤机前的呼吸参数,分析撤机失败组合并症的临床疗效。结果:成功组机械通气时间明显短于失败组,呼吸频率低于失败组,潮气量大于失败组,内源性呼吸末正压低于失败组(P<0.05)。撤机失败组合并症治疗失败前3位分别为营养不良301例(61.0%)、肺部感染控制不力262例(53.1%)、急慢性肾衰竭疗效不良179例(36.3%)。结论:撤机前呼吸功能的改善程度及MV过程中合并症的处理效果是影响患者能否成功撤机的主要因素。Objective：To study the clinical factors of successful weaning that impact mechanical ventilation in the patients of COPD. Methods：1201 patients of COPD that accept mechanical ventilation were divided to two groups ,which concluded successful weaning and unsuccessful weaning.Results：To compare the group of unsuccessful weaning,the time of mechanical ventilation distinctly decreased ,the respiratory rate depressed,the VT accreted,the PEEPi debased in the group of successful weaning （P 〈0.05 ）.The frontal three incorporations were nutrition （61.0%）,failure of controlling lung-infection （53.1%）,bad effect on acute and chronic renal failure （36.3%） ,which were factors of Treatment Failure in the group of unsuccessful weaning.Conclusion：Improvement of respiratory function before weaning and treatment effect to the incorporations during the course of mechanical ventilation ,which were central factors to be or not be able to successfully wean.
