清热燥湿化瘀法对高脂血症患者血脂及动脉弹性指数的影响

摘　　要：目的观察清热燥湿化瘀并治法对高脂血症患者血脂和血管内皮功能的影响。方法将符合入选标准的200例高脂血症患者,分为治疗组、对照A组、对照B组、对照C组及对照D组,共5组,每组40例;分别给予清热燥湿化瘀中药、活血化瘀中药、清热解毒中药、燥湿化瘀中药及氟伐他汀口服,均治疗12周。分别于治疗前后检测血清TC、TG、HDL-c、LDL-c、CRP及动脉弹性指数;同时监测血、尿、粪常规,及肝肾功能、心电图、凝血功能等安全性指标。结果经过12周的临床治疗后,①各组血浆TC、LDL-c水平均较治疗前明显降低(P<0.05),治疗组和对照D组比较差异无统计学意义(P>0.05),优于其他3组(P<0.05);且治疗组和对照D组血浆HDL-c水平升高优于其他3组(P<0.05)。②治疗组、对照B组和对照D组血浆CRP水平均较治疗前降低(P<0.05),且低于对照A组和对照C组(P<0.05)。③治疗组、对照B组和对照D组的大动脉弹性指数(C1)明显高于治疗前(P<0.05)。所有受试者治疗前后安全性指标检测均无异常。结论清热燥湿化瘀并治法可安全有效地降低血脂、抑制炎症因子的形成,保护血管内皮功能,且疗效优于单纯清热解毒法、单纯燥湿化瘀法和单纯活血化瘀法。Objective To observe the effects of heat-clearing, damp-drying and stasis-resolving therapy on blood lipid and artery flexibility index in patients with hyperlipemia. Methods 200 subjects were divided into 5 groups： treatment group（heat-clearing ,damp-drying and stasis-resolving group）, control group A（blood-activating and stasis-resolving group） ,eontrol group B（ heat and detoxication-clearing group） , control group C（ damp-drying and stasis-resolving gruop） and control group D（fluvastatin）, 40 cases in each group. The concentrations of TC, TG, HDL-C, LDL-C ,CRP and the artery flexibility index were measured before and after the treatment; blood routine, urine routine, stool routine, liver funetion, kidney function, ECG and the blond clotting function were monitored also. Results After 12-week treatment, the concentrations of TC and LDL-C in all groups decreased obviously（ P 〈0. 05-0. 01 ） ; the treatment group and control group D had the considerable effect（P 〉 0.05 ） , significantly better than the other 3 groups（ P 〈0.05 ） ; and levels of HDL-c in the treatment group and control group D were higher than those in the other groups（ P 〈0.05 ）. The levels of CRP in the treatment group and the control groups B and D were lower than those before treatment, also lower than those in the control groups A and C （P 〈0.05 ）. The flexibility indexes C1 of the main artery in the treatment group and the control groups B and D were significantly higher than those before treatment（P 〈0.05 ）. All the safety indexes in all the groups were in normal range. Conclusion Heat-clearing, dampdrying and stasis-resolving therapy may safely reduce the blood lipid, inhibit the generation of inflammatory factors, protect the function of artery endothelium, and the effects were significantly better than the single therapy of heat and detoxication-clearing, damp-drying and stasis-resolving, blood-activating and stasis-resolving therapies.
