Ⅲ度碱烧伤创面切削痂与植皮时机的选择

摘　　要：目的探讨Ⅲ度碱烧伤手术切削痂范围与植皮时机。方法对Ⅲ度碱烧伤患者40处创面治疗进行分析。结果40处Ⅲ创面其中15处传统切削痂植皮,术后20～25d愈合,另25处Ⅲ创面行早期切削痂后辐照猪皮覆盖,延迟48h植皮,术后10～15d愈合,平均愈合时间提前7～9d,(P<0.01,具有统计学意义)。结论Ⅲ度碱烧伤创面应采取切削痂延迟植皮方法,虽然为两次手术,但每次手术时间短,出血少,植皮成活率高,创面恢复效果好。Objective To investigate the cutting scab and escharectomy area and the choice of opportunity of skin grafting on III degree alkali burns wounds patients.Methods 40 alkali burnt wounds were randomly divided into control group （n=15） performed skin graft immediately after cutting scab and escharectomy and experimental group （n=25） performed skin graft delayed 48 hours after cutting scab and escharectomy,at the same time, the wounds in the experimental group were covered using pigskin force-exposed with 40w ultraviolet ray after cutting scab and escharectomy, analyzing the average healing time of 40 III 0 alkali burnt wounds.Results The average healing time was 20- 25days in the control group wounds, compared with the control group wounds, the average healing time in the experimental group wounds was 10- 15days, the average healing time was less 7- 9 days （P〈0.01）than that in the con- trol group .Conclusion On the III degree alkali burns wounds,immediately cutting scab and performing skin graft delayed 48 hours after cutting scab and escharcctomy is a better treatment method than immediately cutting scab and immediately performing skin graft after catting, because the time of operation is shorter, the volume of blooding is less, the skin survival rate is higher and the effect of wounds healing is better.
