血液病患者深部曲霉菌感染的血清学诊断

摘　　要：目的:评价半乳甘露聚糖抗原(GM)检测和(1,3)-β-D-葡聚糖(BG)检测对血液病患者侵袭性曲霉菌感染(IA)的诊断价值。方法:检测252例血液病患者血浆BG和血清GM水平,比较BG检测和GM检测的敏感性、特异性、阳性预测值(PPV)、阴性预测值(NPV)。结果:BG检测对IA的敏感性为80.0%,特异性为75.0%,PPV为61.5%,NPV为88.2%。GM检测对IA的敏感性为91.7%,特异性为89.2%,PPV为80.9%,NPV为95.5%。结论:GM和BG检测对于血液病患者侵袭性曲霉菌感染有较好的敏感性和特异性,联合检测是早期诊断IA的较好指标。Objective：To investigate the clinical significance of galactomannan（GM） and（1,3）-β-D-glucan（BG） detection in the diagnosis of invasive Aspergilosis infection（IA） in patients with hematological malignancies.Methods：According to results of GM and BG detection for 252 patients with hematological malignancies,we can achieved the sensitivity,specificity,positive predictive value（PPV）,negative predictive value（NPV） of GM and BG.Results：The BG detection for diagnosis of IA with plasma has shown sensitivity,specificity,PPV and NPV values of 80.0%,75.0%,61.5%,88.2%,respectively.The GM detection for diagnosis of IA with serum has shown sensitivity,specificity,PPV and NPV values of 91.7%,89.2%,80.9%,95.5%,respectively.Conclusion：The detection of GM and BG have shown high specificity and sensitivity for screening high-risk haematological patients.The combined measurement of GM and BG appear to be useful for early diagnosis of IA.
