摘 要:目的:了解下呼吸道感染产超广谱β-内酰胺酶(extended-spectrum beta-lactamases,ESBLs)菌的流行情况以及耐药现状,为临床合理经验性应用抗菌药物提供依据。方法:用WHONET5.3软件统计分析2004~2008年南京医科大学第一附属医院患下呼吸道感染的住院患者痰液标本培养及药敏结果。结果:下呼吸道感染病原体中,革兰阴性菌居首位,产ESBLs菌株比例较高,近5年检出率在14.3%~25.0%之间,对3、4代头孢菌素及氟喹诺酮类耐药比例较高。结论:目前下呼吸道感染病原体中产ESBLs菌比例较高。针对产ESBLs菌株建议首选碳青霉烯类,其他药物耐药率较高。Objective:To investigate the prevalence of extended-spectrum beta-lactamases (ESBLs)-producing pathogens in lower respiratory tract infection and the situation of drug resistance to provide evidence for right empiristic clinical therapy. Methods:The cultured results of sputum specimens isolated from inpatients with lower respiratory tract infection in the First Affiliated Hospital of NJMU from 2004 to 2008 and the drug susceptibility of pathogens were analyzed by the software WHONET5.3. Results:Among all pathogens,gram-negative bacteria led the list while bacteria producing ESBLs accounted for 14.3%~25.0% during these five years, with high rate of resistance to third and forth generation Cephalosporins and Fluoroquinolones. Conclusion:At present,the percentage of ESBLs-producing pathogens in lower respiratory tract infection was high. Carbopenems was recommended because resistance to other antibacterial agents was obvious.
