辛伐他汀对慢性心力衰竭患者心功能和CRP水平的影响

摘　　要：目的:探讨辛伐他汀对慢性心力衰竭患者心功能及CRP水平的影响。方法:118例慢性心力衰竭患者随机分为治疗组和对照组,均给予常规抗心衰治疗,治疗组加服辛伐他汀,疗程6个月。治疗前后评估NYHA分级,用彩色多普勒测定心功能指标[左室射血分数(LVEF)、左室收缩末容积指数(LVESVI)、左心室舒张末容积指数(LVEDVI)、E/A比值],测定C-反应蛋白(CRP)水平。结果:治疗后治疗组总有效率明显高于对照组,心功能NYHA分级明显改善(P<0.01),心功能指标LVEF、E/A比值明显升高,LVEDVI、LVESVI及CRP水平明显下降(P<0.05)。治疗组血浆CRP水平与LVEDVI、LVESVI呈正相关(r=0.31,P<0.05),与LVEF、E/A比值增加呈负相关(r=-0.387,P<0.05)。结论:辛伐他汀治疗慢性心衰效果确切,其主要作用机制为降低血浆CRP水平及其他炎症介质。Objective：To observe the effect of Simvastatin on cardiac function and C-reaction protein （CRP） in patients with chronic heart failure （CHF）. Methods：118 patients with CHF were randomly divided into treatment group and comparison group. All patients were given basic treatment. The treatment group was added with Simvastatin for 6 months. The serum level of CRP was measured before treatment and 6 months after treatment. The left ven tricular ejection fraction （LVEF）,left ventricular end-diastolic volume index （LVEDVI）,left ventricular end-systolic （LVESVI） and E/A ratio were ex-amined by echocardiography. Results：The efficiency rate of treatment group was higher than that of the comparison group,the NYHA classification of cardiac function was obviously improved after 6 months＇ treatment （P0.01）. The LVEF and E/A ratio of the treatment group increased obviously,LVEDVI,LVESVI and serum CRP of the treatment group decreased obviously compared with that of the comparison group,the difference was significant （P0.05）. The serum level of CRP was positively correlated with LVEDVI and LVESVI （r=0.31,P0.05）,the serum level of CRP was negatively correlated with LVEF and E/A ratio （r=-0.387,P0.05）. Conclusion：Simvastatin has the determinate effect in the treatment of patients with CHF,its main mechanism is reducing the serum CRP and other inflammatory media.
