改良甲皮瓣游离移植在拇指脱套伤及拇指再造中的应用

摘　　要：目的:探讨改良甲皮瓣游离移植在拇指脱套伤及拇指再造中的临床应用效果。方法:对13例拇指脱套伤及拇指缺损患者应用改良甲皮瓣游离移植修复。结果:术后13例皮瓣全部成活。修复后的拇指外形逼真,感觉及运动功能均达优良,并保留趾,减少对足的破坏。趾跖底负重面舌状皮瓣均成活,植皮成活,外观饱满。供足行走,跑步无明显异常。随访2～15个月未发现趾植皮失活、破损。结论:采用改良甲皮瓣游离移植是拇指脱套伤修复及拇指缺损再造较好的方法之一。Objective：To evaluate the clinical outcome of modified wrap-around flap for thumb reconstruction and degloving injury of the thumb. Methods：Thirteen cases of thumb defect and degloving injury of the thumb were treated with modified wrap-around flap to reconstruct the thumb. Results：The transferred flaps survived in all cases. The repaired thumb had a good appearance,sensory and movement. The donor big-toe was preserved. The tongue-shaped flaps,which was in the weight bearing area of the tibial side of the big toe,was all survived. Skingrafting was survived. The postoperative gait and running was normal. The patients were followed 2-15 months postoperatively.No failure was occurred in Skingrafting of big-toe. Conclusion：Modified wrap-around flap is one of the good method in the treatment of thumb defect and degloving injury of the thumb.
