糖调节受损者的主要中医体质类型及其与相关指标的关系

摘　　要：目的:探讨糖调节受损(IGR)者的主要体质类型及其与相关指标的关系,为IGR的防治提供科学依据。方法:按照流行病学的抽样要求,在对市区、县、乡镇、村庄抽样的1500例人群,进行葡萄糖耐量试验(OGTT)、胰岛素释放试验等检测的基础上,筛选出符合IGR诊断标准者322例进行中医体质调查并进行分析。结果:①322例IGR人群,单一体质类型者、2种体质类型相兼者、3种体质类型相兼者分别占74.8%、19.5%、8.1%;前5位的体质类型是:平和质130例(40.4%)、气虚质40例(12.4%)、阳虚质24例(7.5%)、气虚兼阳虚质21例(6.5%)、痰湿质20例(6.2%)。②与痰湿质比较,平和质、气虚质、阳虚质、气虚兼阳虚质组的体质量指数(BMI)、腰臀比(WHR)显著较低(P<0.01);而气虚质、阳虚质、气虚兼阳虚质组的BMI、WHR与平和质相比显著较低(P<0.05)。痰湿质的HOMA-IR、HOMA-β均高于平和质、气虚质、阳虚质和气虚兼阳虚质(P<0.01)。结论:①IGR人群体质类型表现为单一体质、2种及3种体质类型相兼出现,IGR人群的主要体质类型是平和质、气虚质、阳虚质、气虚兼阳虚质、痰湿质。②BMI、WHR、HOMA-IR及HOMA-β是IGR各主要体质类型的参考指标。在IGR人群中,气虚质、阳虚质及气虚兼阳虚质的胰岛分泌功能较差;痰湿质的胰岛分泌功能尚可,但胰岛素抵抗较明显。Objective：To confirm the correlations between the soma ofthe traditional Chinese Medicine and the objective index about Homeostasis model assessment-Insulin Resistance index（HOMA-IR）and the function about beta Cell ofislet（HOMA-β）with Impaired glucose regulation（IGR）,to explore the basis for the syndrome ofthe traditional Chinese Medicine and insulin function.Methods：According from the epidemiological investigation,we had a Oral Glucose Tolerance Test and insulin releasing test to the 1 500 persons who came from downtown area、county、small towns、village,in accordance with the soma ofTCM,then investigate and analysis the correlations who meet the diagnosis ofIGR.Results：①In all 322 cases,each ratio ofthe group about signal soma、twice soma、three soma is 74.8%、19.5%、8.1%,Ahead somas are mild, deficiency ofQi, deficiency ofYang, deficiency ofQi and Yang,phlegmatic hygrosis.②The BMI ofmild group is higher than the deficiency ofQi,deficiency ofYang,deficiency ofQi and Yang group（P0.05）;The WHR ofphlegmatic hygrosis group is significantly higher than the mild,the deficiency ofQi,deficiency ofYang,deficiency ofQi and Yang group（P0.01）;The HOMA-IR ofphlegmatic hygrosis group is significantly higher than the mild,the deficiency ofQi,deficiency ofYang,deficiency ofQi and Yang group（P0.01）;The HOMA-β ofmild group is higher than the deficiency ofQi,deficiency ofYang,deficiency ofQi and Yang group（P0.05）.Conclusions：①IGR display the signal soma, twice soma and three soma, The main somas by turns are mild,deficiency ofQi,deficiency ofYang, deficiency ofQi and Yang,phlegmatic hygrosis.②The objective indexes ofthe main soma are BMI,WHR,HOMA-IR,HOMA-β;The insulin secretion ofthe deficiency ofQi,deficiency ofYang,deficiency ofQi and Yang is more lower;The function ofinsulin section ofthe deficiency ofQi phlegmatic hygrosis is fairly,but the insulin resistance is obviously.
