摘 要:目的探讨庚型肝炎病毒(HGV)在维持性血液透析(HD)患者中的流行情况及感染相关因素。方法应用套式逆转录-聚合酶链反应(RT-PCR)方法检测50例HI)患者血清HGV RNA,并分析其与透析时间、输血、肝功能损害及丙型肝炎病毒(HCV)、乙型肝炎病毒(HBV)感染的关系。结果 HD患者HGV感染率达14%,远高于正常对照人群。HGV感染相关因素分析提示;随着透析时间的延长,输血次数的增加,HGV感染率明显增高;同时HGV伴发HCV感染的发病率明显增高(43% vs.5%,P<0.01),并增加了肝功能损害的机率(29%vs 12%,P<0.05)。对HGV基因5’NCR区序列分析发现,7名阳性患者中有6名患者的序列完全相同,另一名患者仅有一个碱基不同。结论 HD患者是HGV感染的高危人群,其高发生率可能与输血有关。HGV多与HCV同时感染,并可能诱发肝损害。HD患者HGV基因有高度同源性,提示有可能为同一病毒株感染所致。Objective To evaluate the prevalence and clinic relevance of hepatitis G virus(HGV)infection in maintenance hemodialysis patients. Methods Reverse-transcription(RT) nested polymerase chain reaction(PCR)was used to detect HGV in 50 HD patients. The prevalence of HGV infection, their relationship with risk factors, liver function and HBV, HCV infection were investigated. Results HGV RNA was found in 14 percent of the HD patients (7 of 50), as compared with none of health blood donors(0 of 20, P<0.01). After long time hemodialysis and many times of blood transfusion, HGV infection was significantly increased. Meantime, 3 of 7 HGV infection patients were also infected with HCV(43%) and 2 patients with HGV infection had a history of increased alanine transferase(ALT)levels. When the 112bp RNA HGV 5' NCR PCR products from 7 HD patients were sequenced, 6 of 7 patients were total homology at the nucleotide level, the another one was just one nucleotide different. Conclusion Patients on maintenance HD are at increased risk of HGV infection. It may be associated with blood transfusion. HD patients with HGV infection are often accompanied by HCV infection and may cause liver damage. The infection of HGV might be caused by same wild virus.
