摘  要:目的探讨初诊2型糖尿病患者慢性并发症与外周血单核细胞血红素加氧酶1(HO-1)表达的相关性。方法初诊2型糖尿病36例分为并发症组和无并发症组,正常对照组(NC)10名,比较各组空腹血糖(FPG)、血清丙二醛(MDA)水平、外周血单核细胞活性氧(ROS)产量及HO-1表达水平,并分析其相关性。结果糖尿病各组的FPG、MDA、ROS和HO-1水平均显著高于NC组(P<0.01),有并发症组的上述各指标均显著高于无并发症组(P<0.05)。HO-lmRNA分别与HO-1蛋白、MDA、FPG水平显著正相关,HO-1蛋白水平与ROS、MDA显著正相关,MDA与ROS显著正相关(P<0.05)。结论初诊2型糖尿病合并慢性并发症患者的高血糖引起氧化应激并诱导HO-1适应性和代偿性增高,将来有望通过适当诱导HO-1高表达以拮抗糖尿病氧化应激。Objective To investigate the relationship between diabetic chronic complications and expression of HO-1 in peripheral blood mononuclear cells (PBMC) in newly diagnosted type 2 diabetie patients. Methods There were two groups of newly diagnosed type 2 diabetic patients, chronic diabetic complication group and non-complication group. Fasting plasma glucose (FPG), HbA1c, serum MDA, ROS output and expression of HO-1 mRNA and protein in peripheral blood monoeytes were determined. Also health volunteers were used as control group. Results Control group had significantly lower levels of FPG, MDA, ROS output, HO-1 mRNA and protein than diabetic group. Complication group had significantly higher levels of FPG (P〈0. 05), HbA1c (P〈0.05), ROS output (P〈0.01), serum MDA (P〈0. 01), and expressions of HO-1 mRNA (P〈0. 05) and protein (P〈0. 05) when compared with non- complication group. There was positive correlation between the expression of HO-1 mRNA and levels of FPG, MDA and HO-1 protein (P〈0.05). Also there were positive correlation between the expression of HO-1 protein and ROS output, serum MDA (P〈0. 05). And MDA was correlated with ROS significantly (P〈 0. 05). Conclusions There are significant oxidative stress in newly diagnosed type 2 diabetic patients with chronic complications. HO-1 may be one of the most important protective mechanisms against oxidative stress induced by diabetes mellitus, and high expression of HO-1 may prevent or prolong the development of diabetic chronic complications.
