摘  要:目的探讨烫伤后不同组织脂多糖结合蛋白(LBP)mRNA的动态表达及其与内毒素血症的关系。方法采用大鼠35%体表面积Ⅲ度烫伤模型,实验动物随机分为正常对照组、烫伤组和多粘菌素B治疗组。血浆内毒素水平测定采用改良过氯酸预处理血浆、偶氮显色法鲎实验定量测定,组织LBPmRNA含量采用RT-PCR法。结果烫伤后门静脉及体循环内毒素水平均显著升高,8小时达峰值。给予多粘菌素B治疗可不同程度地降低血浆内毒素水平。正常组织可表达少量LBP,烫伤后2小时不同组织LBPmRNA表达较对照值有显著升高(P<0.05~0.01),8小时达峰值,约为对照值的2~2.5倍,且一直持续至伤后24小时。给予多粘菌素B抗内毒素治疗,可不同程度抑制肺、肠、肾组织LBPmRNA的表达(P<0.05~0.01)。相关分析显示,门静脉内毒素水平与肺组织LBPmRNA呈显著正相关(r=0.412,P<0.05),但与肝、肠、肾组织无显著相关性。结论组织LBPmRNA表达在烧伤后显著增多。Aim To observe the dynamic changes of mRNA expression in lipopolysaccharide binding protein (LBP) and the relationship between LBP mRNA and gut derived endotoxemia after thermal injury. Methods Fifty six male Wistar rats were subjected to a 35% total body surface area full thickness thermal injury, and randomly divided into the normal control group, 35% Ⅲ° injury group and the polymyxin B treatment group. Plasma endotoxin was determined by limulus amebocyte lysate, and the tissue LBP mRNA expression was measured by reverse transcription polymerase chain reaction (RT PCR). Results It was found that a significant elevation of endotoxin concentration in both of portal and systemic circulation was observed at 2 hours after burns, peaking at 8 hours ( P < 0.05  ̄0.01) and decreasing at 24 hours. LBP mRNA expression significantly increased in liver, lung, intestine, and kidney at 2 hours, peaking at 8 hours and sustaining a high level till 24 hours. However, treatment with polymyxin B could markedly reduce both of the portal and systemic endotoxin levels and LBP mRNA expression in various organs. Moreover, a positive correlation between plasma endotoxin value and pulmonary LBP mRNA expression was noted ( r =0.421, P <0.05). Conclusion This study suggests that a marked elevation of LBP mRNA expression is evident in multiple organs, which might be associated with gut derived endotoxemia following thermal injury.
