中学教师原发性高血压的动态血压研究  

STUDY ON AMBULATORY BLOOD PRESSURE IN ESSENTIAL HYPERTENSION OF MIDDLE SCHOOL TEACHERS

摘  要:目的:探讨工作比较紧张的中学教师在工作、教学、运动及休息时的动态血压变化。方法:采用SpacelabsABP监测(ABPM)仪。配带24小时,自动充气测量。记录收缩压(SBP)、舒张压(DBP)、平均动脉压、心率等数值,有效记录要求在80%以上。结果:24小时动态血压均值为19.39±2.39/11.99±1.12kPa。夜间血压负荷SBP、DBP与白昼相比差异显著,P<0.01,DBP血压负荷与SBP相比差异显著,P<0.01。另外,动态血压昼夜节律明显异常(77%夜间血压下降率<10%),夜间平均血压高于白昼者15%。结论:教师原发性高血压昼夜节律明显异常,波动幅度减小甚至消失。这些改变除教师繁重的工作外,与生物学、遗传学、行为及社会因素有一定关系。Objective:To investigate variation of ambulatory blood pressure(ABP) in middle school teachers during various kinds of activities such as work,teaching,movement and rest.Methods:Spacelabs monitoring instrument was used all day long for automatic recording.The recording indexes include:systolic blood pressure(SBP),diasolic blood pressure(DBP),mean artery pressure,heart rate and so on.The qualified recordings were over 80%.Results:The mean blood pressure value in 24 hours was 19.39 ± 2.39 / 11.99 ± 1.12 kPa.The load SBP and DBP showed significant differences between the nocturnal and diurnal time(P< 0.01 ).The variation between DBP and SBP load was significant(P< 0.01 ).Besides,the nocturnal and diurnal rhythm of ABP showed obvious abnormalities (In the nocturnal time,77% showed decrease less than 10% of the blood pressure.The mean blood pressure of the nocturnal time was higher than that of the daytime,accounting for 15% of the blood pressure.Conclusion:The nocturnal and diurnal rhythm in teachers with essential hypertension shows clear abnormalities,the wave altitude becomes less or disappears.All these changes relate to intense and heavy work of the teachers,and in certain degree correlate with the biology,genetics,behaviour and social factors.
