摘  要:目的全面系统地观测正常成年人半月板与猪半月板的组织及解剖形态,分析其差异性及异种半月板移植形态学上的可行性。方法对正常成人半月板与成年猪(体重90~120kg)半月板进行大体解剖学测量,包括半月板前后角、体部的宽度和厚度,半月板的矢状径、横径及组织进行免疫组化光学观察和透射电镜观察,对数据进行详细记录,数据处理均采用SPSS11.5统计分析软件,两两比较采用两独立样本的t检验。结果猪半月板与人半月板外形基本相似,内侧呈"C"形、外侧呈"O"形。人半月板前后角止于胫骨平台,而猪外侧半月板后角止于股骨后髁。人内侧半月板的最大矢状径、最大横径均比外侧半月板大,而猪外侧半月板最大矢状径、最大横径均比内侧半月板大。与人半月板比较,猪内侧半月板的前角、体部、后角宽度无显著差异(P>0.05),而外侧半月板具有显著差异(P<0.05),内、外侧半月板前角、体部、体部中点、后角各项指标的厚度均有统计学差异(P<0.05),内侧半月板的最大矢状径、最大横径具有显著差异(P<0.05),外侧半月板最大矢状径、最大横径无显著差异(P>0.05)。猪半月板与人半月板组织学上无显著差异(P>0.05)。结论由于实验样本来源的限制,本实验研究结果表明并不能为成人半月板移植提供完全匹配,或许可以为青少年半月板移植提供匹配,对异种半月板移植的可行性具有一定的指导意义及参考价值。Objective To observe normal human meniscus and porcine meniscus with the tissue and anatomy comprehensively and systematically and analyze the differences and viability of morphology feasibility of the xenogeneie meniscus transplantation. Methods The meniscus of normal human adults and adult pigs(weight 90-120 kg) underwent general anatomy measurement, including meniscus width and thickness of the anterior horn, posterior horn,body and meniscus sagittal diameter and transverse diameter.The meniscus tissue also received immunohistochemical optical observation and TEM observation. The data were recorded detailedly.The statistical analysis software SPSS l 1.5 was used to process the data, and two independent samples t test was used to compare the couple data.Results Porcine meniscus and human meniscus had the similar shape.The lateral meniscus was similar to "O"-shape and the medial meniscus in a "C"--shape.The human's meniscus terminated at tibial plateau while the porcine lateral meniscus posterior horn terminated after femoral condyle.Human medial meniscus's largest sagittal diameter and the maximum diameter were bigger than the lateral meniscus. However, the porcine medial meniscus was contrary.Compared with human, the porcine medial meniscus width of the anterior horn,the body, posterior horn had no significant differences (P〉0.05) while the lateral meniscus had significant differences (P〈0.05).The porcine meniscus thickness of the anterior horn,body, body midpoint,posterior horn was significantly greater than colTesponding human meniscus's (P〈0.05). The largest sagittal diameter,maximum diameter of medial meniscus had significant difference(P〈0.05).However,the lateral meniscus in the largest sagittal diameter,maximum diameter had no significant difference(P〉0.05).Porcine meniscus compared with the human's in the morphology had no significant difference (P〉0.05).Conclusion This experiment's results show that it can not provide perfectly matched meniscus
