需氧菌阴道炎菌群及治疗效果的临床研究

摘　　要：目的:研究需氧菌阴道炎(AV)患者阴道菌群情况,观察复方莪术油栓剂及甲硝唑的疗效。方法:由全国6家医院按AV标准诊断AV80例,前瞻性随机对照比较复方莪术油栓剂与甲硝唑的疗效。结果:(1)AV患者多有外阴灼痛、性交痛、白带增多等症状,阴道黄色分泌物占87.0%,清洁度Ⅲ度占85.0%,pH>6占77.2%,外底层细胞比例>10%18例,占22.5%;(2)AV是以大肠杆菌为主,其次为粪肠道球菌、棒状杆菌、金黄色葡萄球菌等需氧菌感染的有明显炎症表现的阴道炎症;(3)治疗7天时复方莪术油栓剂组与甲硝唑组治疗AV的总有效率分别为86.96%与79.41%(P>0.05)。复方莪术油栓组外阴灼痛的症状比甲硝唑组明显改善(P<0.05);停药30天外阴灼痛症状及分泌物形状复方莪术油栓组比甲硝唑组改善明显(P=0.003及0.033),总有效率明显高于甲硝唑组(93.48%vs61.76%,P=0.001)。结论:AV是一种由需氧菌引起的阴道炎症,用复方莪术油栓剂治疗AV疗效明显优于甲硝唑。Objective ： To investigate the vaginal flora and treatment outcome of aerobic vaginitis （AV） and observe the effect of FufangEshuyouShuan for AV. Methods： A muhicenter, prospective, case control clinical study was carried out, FufangEshuyouShuan was used in 46 cases with AV as study group, and metronidazole was used in 34 cases as control group. Symptoms, signs, examination of laboratory and curative effect were compared between the two groups. Results：（ 1 ）A burning pain, dyspareunia and leukvrrhagia were complained by about half of patients with AV. Yellowish vaginal secreta（ 87.0% ）, microscopic saline examination of vaginal discharge Ⅲ degree （ 85.0% ）, pH〉6 （77. 2% ）,proportion of parabasal epithelial cells 〉10% （25.5%） were observed. （2） Escherichia coli. corvnebacteriaceae, enterococcus faecalis, staphylococcus aureus, streptococcus heamolyticus, enterobacteriaciae, streptococcus aglactiac and staphylococcus epidermidis were the most common bacteria isolated. （3）The efficiency of FufangEsh-uyouShuan-and metronidazole group were 86.96% and 79.41% respectively （ P〉0. 05 ） after 1 week of treatment,93.48% and 61.76% （P =0. 001 ） after 4 week of treatment.Conclusion：As an independent vaginitis, we should improve the knowledge of AV, and FufangEshuyouShuan shows good effect for aerobic vaginitis than metronidazole.
