摘　　要：介绍了慢性病监测管理信息化在国内、外的发展及现状,并从标准化、整合化和互动化三方面对其未来发展趋势作了探讨;指出标准化将是慢性病监测信息化未来发展的重要方向,而社区卫生服务信息平台与慢性病专病管理系统则将成为慢性病防控的主要模式,并在此基础上与病人实现互动,充分体现"以人为本"的理念。It introduces the development and status in quo of informationization of monitoring and control of chronic disease at home and abroad, and discusses its future development from three aspects, namely, standardization, integrality, and mutuality. It points out that the standardization is an important direction for its future development, and the community health service information platform and the special control system of chronic disease will be the major mode for prevention and control of chronic disease; besides, based on it, the interaction is also realized between doctor and patient, which fully embodies the idea of ＂people-oriented＂.
