摘 要:目的:观察骨碎补总黄酮对去势骨质疏松动物模型假体周围骨组织形态的影响,探讨其在防治人工关节假体无菌性松动的作用。方法:将32只雌性大白兔随机分空白组、模型组、骨碎补总黄酮组、阿仑膦酸钠组,除空白组外,其余3组切除双侧卵巢,成功建立骨质疏松模型后,将氧化锆陶瓷假体植入四组动物模型左侧胫骨上段髓腔,分别以相应药物灌胃。于给药前和给药12周后行假体周围感兴趣区骨密度测定,所有动物于12周后处死,行骨组织形态计量学测定和分析。结果:骨碎补总黄酮组假体周围局部感兴趣区骨密度较对照组明显升高,差异有统计学意义(P<0.01),而与阿仑膦酸钠组比较差异无统计学意义(P>0.05);骨组织形态计量学参数结果显示:骨碎补总黄酮组中反应骨量和骨微结构的参数(BV/TV、Tb.Th、Tb.N、Tb.Sp)较模型组均有明显改善,其差异有统计学意义(P<0.01),反应骨吸收的(Oc.No/Tb.PM、ES/BS)较模型组显著降低(P<0.01),而反应骨形成的参数(%L.Pm、OS/BS、MAR、BFR/TV)较阿仑膦酸钠组显著增高(P<0.01或P<0.05)。结论:骨碎补总黄酮可明显抑制人工关节假体周围骨吸收,减少骨量丢失,促进骨形成,增加假体周围骨密度,促进生理性骨矿化,改善骨质量和骨微结构,具有明显的防治骨质疏松兔假体松动的作用。Objective:To observe the effect of rhizoma drynariae flavones (RDF) on the bone histomorphology around the joint prosthesis (JP) in ovariectomized osteoporosis animal model,and to investigate the prevention and treatment of the aseptic loosening of prosthesis by RDF. Methods:Thirty-two female rabbits were randomly divided into control group,model group,RDF group and alendronate group. The rabbits in all groups except the control group received resection of bilateral ovarians to establish osteoporotic models. After 24 weeks,the osteoporosis model was successfully established,and then the rabbits in all groups were implanted zirconia ceramic prosthesis into the left tibial marrow cavity. The rabbits in RDF group and alendronate group were given RDF powder and Fosama,respectively. Meanwhile,the rabbits in the control group and model group were given distilled water. The bone mineral density (BMD) of the region of interest (ROI) was measured before treatment and also 12 weeks after treatment. Histomorphometry of bone tissue around the prosthesis was observed 12 weeks after administration. Results:The BMD of ROI around the prosthesis in RDF group was significantly higher than that in the model group (P〈0.01). And there was no statistical difference of BMD between RDF group and alendronate group (P〉0.05). Bone histomorphometry showed that bone loss and microstructure profile (BV/TV,Tb.Th,Tb.N,and Tb.Sp) in RDF group were remarkably improved compared with those in the model group (P〈0.01); the indexes of bone resorption (Oc.No/Tb.PM and ES/BS ) obviously decreased compared with the model group (P〈0.01); the indexes of bone formation (%L.Pm,OS/BS,MAR,and BFR/TV) in RDF group evidently increased compared with the alendronate group (P〈0.01 or P〈0.05). Conclusion:RDF can inhibit bone resumptions evidently,promote bone formation,increase the bone density and improve bone mass around joint prosthesis.
