鼻内镜下两种微创手术方法处理下鼻甲肥大效果的比较

摘　　要：目的探讨下鼻甲肥大的手术治疗方法并评价其效果。方法在鼻内镜下分别应用半导体激光和电动切割器对37例共74侧肥大的下鼻甲进行微创手术治疗,观察术中出血情况和术后鼻腔通气功能改善情况,并随访1年以上,评价其疗效。结果应用激光处理下鼻甲术中出血量平均为5.6mL,应用电动切割器则平均为19.3mL,两者之间的差异有显著性(P<0.05);1个月后的有效率前者为67.57%(25/37),后者为70.27%(27/37),两者之间的差异无显著性(P>0.05);1年后的有效率前者为94.59%(35/37),后者为59.46%(22/37),两者之间的差异有显著性(P<0.05)。结论鼻内镜下微创手术处理下鼻甲肥大,选择半导体激光较电动切割器可明显减少术中出血量,且远期效果前者优于后者。[ Objective ] To compare the effectiveness of semiconductor laser and electromotive cutterbar in nasal endoscope for patients with hypertrophic inferior turbinates. [Methods] A total of 74 cases with hypertrophic inferior turbinates were randomly assigned into two groups of 34, patients in one group were microinvasively treated by semiconductor laser under nasal endoscope, those in the other were microinvasively treated by electromotive cutterbar under nasal endoscope. The blood loss during operation and the ventilation function of nasal cavity were observed, and all patients were followed up for more than 1 year, and the effect was evaluated. [Results] The average blood loss was 5.6 mL in semiconductor laser treated patients, that in electromotive cutterbar treated patients was 19.3 mL, there was significant deviation between the two groups. One-month efficient rate in the former was 67.57% （25/37）, that in the latter was 70.27% （27/37）, and there was no significant deviation（Chi-square test,P〉 0.05）. One-year efficient rate in the former was 94.59% （35/37）, that in the latter was 59.46% （22/37）, there was significant deviation （Chi-square test, P 〈0.05）. [ Conclusion ] The nasoendoscopic microsurgery for hypertrophic inferior turbinates can conspicuously reduce the blood loss by using semiconductor laser, and its long-term effectiveness is better than that by electromotive cutterbar.
