缬沙坦逆转高血压性心肌肥厚的临床研究

摘　　要：目的观察缬沙坦对原发性高血压（EH）患者左室肥厚（LVH）和心肌纤维化的影响。方法58例经确诊为EH伴LVH患者,服用缬沙坦治疗24周。治疗前、后分别测算测量舒张末期室间隔厚度（IVST）、左心室后壁厚度（LVPWT）及左心室舒张末期内径（LVEDd）,测定血清Ⅲ型前胶原（PCⅢ）、透明质酸（HA）、层黏蛋白（LN）、Ⅳ型胶原（CⅣ）。并与对照组（30例）比较。结果高血压左室肥厚组CⅢ、HA、LN、CⅣ均高于对照组,经24周治疗后,血清PCⅢ、HA、LN、CⅣ较对照组显著下降,同时左室肥厚状况也显著改善。结论缬沙坦可以减轻EH患者心肌纤维化程度而逆转高血压性心肌肥厚。Objective To detect the effect of valsartan to essential hypertension （EH） patient with ventricular hypertrophy （LVH） and cardiac muscle fibrosis. Methods Fifty-eight cases of EH combined with the LVH,take valsartan for 24 weeks. The measurements of interventricular septal thickness （IVST）,left ventricular posterior wall thickness （LVPWT） and left ventricular end-diastolic dimension （LVEDd）,were taken pre-and post treatment. The concentration serumⅢPrecollagen （PC Ⅲ）,Hyaluric acid （HA）,level mucoprotein （LN）,ⅣCollagen （C Ⅳ） were also detected and compares with the control group （30 examples）.Outcomes In group of EH with LVH,the concentration of CⅢ,HA,LN and CⅣ are significantly higher than the control group.Through 24 weeks treatments,serum PC ⅢHA,LN,CⅣdrop remarkable,at the same time the condition of LVH also obviously improves. Conclusion Valsartan may reverse LVH induced by EH through reduction the cardiac muscle fibrosis.
