不同强度针刺“内关”对心肌缺血再灌注损伤大鼠血清IMA及下丘脑、脊髓5-HT表达的影响

摘　　要：目的观察不同强度针刺对心肌缺血大鼠血清IMA及下丘脑、脊髓5-羟色胺(5-HT)表达的影响及作用机制;并分析比较不同强度针刺内关对心肌缺血疗效的差异。方法将50只SD大鼠随机分为空白组、假手术组、模型组、轻手法针刺组、重手法针刺组。采用冠状动脉结扎法建立心肌缺血再灌注模型,运用721分光光度计测定血清缺血修饰白蛋白含量,免疫组化法测定脊髓背角和背根神经节5-HT阳性表达。结果模型组大鼠心肌缺血再灌注后心电图ST段电位值、血清IMA含量、5-HT阳性表达均较空白组和假手术组明显升高,差异有统计学意义(P<0.01),两个针刺组与之比较显著降低,但仍高于空白组和假手术组,差异有统计学意义(P<0.01);重手法针刺组与轻手法针刺组比较,差异有统计学意义(P<0.05)。结论针刺内关穴能降低MIRI大鼠血清IMA含量,降低MIRI大鼠下丘脑、脊髓背角5-HT阳性表达,抑制应激反应的发生,减少心肌梗死面积,从而起到保护心肌的作用,且重手法针刺组的疗效优于轻手法针刺组。Objective To explore the mechanism of acupuncture on protecting MIRI by observing the effects of different acupuncture intensities on IMA in the serum and the expression of 5-HT in the hypothalamus and the spinal cord in rats with MIRI and compare the differences of protective effect by stimulating Neiguan point among different stimulating intensities. Methods 50 SD rats were randomly divided into 5 groups： blank group, sham operation group, model group, weak acupuncture intensity group and strong acupuncture intensity group. The MIRI model was set by coronary ligation. The IMA in the serum was detected with 721 Spectrophotometer. And the positive expression of 5-HT in spinal dorsal horn and dorsal root ganglion was measured with immunohistochemistry. Results The value of ST in ECG, the level of IMA and the positive expression quantity of 5-HT in model group were higher obviously than those in the blank and sham operated groups （P〈0.01）, and in two acupuncture groups they were decreased obviously, but higher than those in the blank and sham operated groups（P〈0.01）, In the strong acupuncture intensity group, all indices were lower than those in the weak acupuncture intensity group （P 〈0.05）. Conclusions Acupuncture in Neiguan point can decrease the level of IMA and the positive expression quantity of 5-HT in rats with MIRI, restrain the stress reaction and reduce the myocardial infarction size to protect the heart muscles. Meanwhile, the acupuncture effect in the strong acupuncture intensity group is superior to that in the weak acupuncture intensity group.
