摘  要:目的:探讨醛固酮合酶(CYP11B2)基因-344C/T的多态性与心房颤动(Af)的关系。方法:研究对象均来自湖北地区汉族人群,包括120例Af患者,120例非Af者。采用成组配比研究,取静脉血,提取基因组DNA,采用聚合酶链反应-限制性酶切片段长度多态性(PCR-RFLP)分析技术对2组人群CYP11B2基因-344C/T的多态性进行分析。结果:CYP11B2-344CT+CC基因型频率在Af组与对照组之间差异有统计学意义(53.4%∶37.5%,P=0.037),等位基因在2组间亦存在同样的趋势(C/T=28.8%∶19.6%,P=0.019)。单因素非条件Logistic回归分析CT+CC基因型频率患Af的危险性高(OR=1.82,95%CI1.02~3.22,P=0.04),排除混杂因素后,CT+CC基因型与人群患Af的风险呈弱相关(OR=1.73,95%CI0.99~3.02,P=0.056);排除混杂因素后,左房内径与人群患Af的风险总是呈显著相关(OR=8.14,95%CI3.43~19.31,P=0.000)。结论:在湖北地区汉族人群中,CYP11B2-344C/T的点突变与Af的发病呈弱相关性,有可能是Af的遗传危险因素,左房内径的增加与Af的发病呈显著相关。Objective:This study aimed to investigate the relationship between the polymorphism oi the aldo-sterone synthase (CYP11B2) gene --344 C / T and atrial fibrillation. Method:All human came from the Han population in Hubei Province, 120 cases were atrial fibrillation patients, 120 were non-atrial fibrillation. Vein blood were acquired to extracte genomie DNA, and polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP) was used to analyse the polymorphism of the aldosterone synthase (CYPllB2) gene --344C/T analysis in two populations. Result:There was a significant difference between the ease group and control group in CC +CT frequencies of genotype CYPllB2 - 344 C/T polymorphism (respectively 53.4% : 37.5%, P = 0. 037). There was also the same trend between alleles(C/T=28.8%: 19.6%, P=0. 019). The crowd carrying CT+CC genotype suffered from the high risk of AF by single factor non conditional Logistic regression analysis (OR=1.82, 95% CI= 1.02-3.22,P=0.04), however, after exclusion of confounding factors, there were weak association for the people carrying CT+CC genotype suffering from the risk of atrial fibrillation (OR= 1.73,95 % CI = 0.99-3.02,P=0. 056). After exclusion of confounding factors, there were significantly association between left atrial diameter and the risk of suffering from atrial fibrillation(OR = 8.14, 95% CI = 3.43- 19.31, P = 0. 000). Conclusion:Among the Han population in Hubei Province, CYP11B2 --344 C/T gene polymorphisms is weakly associated with atrial fibrillation, which may also be a risk factor of atrial fibrillation, increase in left atrial diameter is significantly associated with the incidence of atrial fibrillation.
