多层螺旋CT血管造影在颅内动脉瘤诊断中的临床价值

摘  要:目的探讨MSCTA在颅内动脉瘤中的临床应用价值及全面、合理评价颅内动脉瘤的影像学方法。方法收集在本院行MSCTA检查的颅内动脉瘤36例,使用PhilipsLightspeed 16CT机,图像经二维及三维后处理得VR、MPR、MIP图像。其中16例行DSA检查,并比较两者图像上的差异。结果MSCTA立体、直观脑血管空间解剖细节,可充分观察到动脉瘤的准确起源、瘤体大小、颈部宽窄及走向等。35例中与手术及DSA对照,准确率达100%。三维成像中以容积显示图像为最佳。3D-MSCTA与3D-DSA比较,在显示动脉瘤形态及瘤颈方面无明显差异。结论MSCTA是一种无创性成像方法,在颅内动脉瘤诊断及术前、术后评价中发挥着重要作用,可以作为动脉瘤的首选检查方法。Objective To discuss the clinical value of multi-slice helical CT angiography(MSCTA)in detection of cerebral aneurysm and the medical imaging method of evaluating cerebral aneurysm generally and rationally.Methods Selecting 36 new cases in our hospital concerning cerebral aneurysm,imaging with Philips Lightspeed 16 CT.Scanning images were transferred into work-station to create two and three-dimensional cerebral vascular images(VR、MIP、MPR images),of which 15 cases were examined by DSA.Compared the differences between the images of MSCTA and DSA.Results MSCTA could clearly display spacial anatomical details so that we can accurately observe the source,size and neck width,route of cerebral aneurysm.36 cases are in accordance with operations or DSA results.The diagnostic accuracy was100%.Volume rendering was the best in the three-dimensional images.There was no difference between the images of MSCTA and DSA in displaying siqure and the neck of aneurysm.Conclusion As a harmless examination method,MSCTA plays an important role in detection,pre-operational and post-operational evaluation of cerebral aneurysm,and can be used as the first examination method.
