离体大鼠心肌对异氟烷预处理保护效应的记忆时间研究

摘　　要：目的应用Langendorff模型研究离体大鼠心肌对异氟烷预处理（isoflurane preconditioning，IsoP）保护效应的记忆时间。方法采用Langendorff离体心脏灌注模型，将24只SD大鼠随机分为4组，每组6只，分为缺血，再灌注损伤组（IR组）、记忆时间1组（M1组）、记忆时间2组（M2组）和记忆时间3组（M3组）。监测复灌后心功能、生化指标和心肌存活面积的变化。结果与IR组相比，M1、M2和M3组左室舒张末压（left ventriculor end-diastolic pressure，LVEDP）均明显降低（P〈0．01），M1和M2组的左心室发展压（left ventriculor develop pressure，LVDP）恢复百分比在复灌30、60min时明显增高（P〈0．05），M2和M3组左室内压下降最大速率（1dp／dk）在复灌后60min明显增高（P〈0．05），M1和M2组在复灌后30、60min的LVDPxHR的恢复百分比高于IR组（P〈0．05），M3组在复灌后60min高于IR组（P〈0．05），M1组LDH在复灌后60min的释放量低于IR组（P〈0．05），复灌5min起肌酸激酶的释放量低于IR组（P〈0．05），M1和M2组的心肌存活面积高于IR组（P〈0．05）和M3组（P〈0．05）。结论离体大鼠心肌对IsoP保护效应的记忆时间在20min达到高峰，随后明显减弱甚至消失。Objective To investigate the memory phase of the protective effect of isoflurane preconditioning （IsoP） on rat myocardium. Methods 24 isolated SD rat hearts were divided into four groups： IR, M1, M2 and M3 groups （n=6） according to the time of washout before ischemia. After equilibration, the hearts were perfused for 20 min, 10 min and 0 min in M1, M2 and M3 groups respectively, using Langen dorff apparatus, then received two cycles IsoP and followed by 10 min, 20 min and 30 min washout respectively. All the hearts were subjected to 30 min sustained ischemia and 60 min reperfusion. LVEDP, LVDP, and-dp/ dtmax were recorded. The release of CK and LDH in the outflow and the size of survival myocardium area were measured. Results During reperfusion, the recovery of LVEDP was lower in M1, M2 and M3 groups than that in IR group （P〈0.01）. LVDP in M1 and M2 groups increased （P〈0.05）. The percentage of recovery of LVDPxHR in M1 and M2 groups at 30 min and 60 min of reperfusion were higher than that in IR group（P〈0.05）. The release of CK in memory groups significantly decreased（P〈0.05）and the contents of LDH in M1 group was lower than other groups （P〈0.05）. The percentage of survival area of myocardium in M1 and M2 groups was higher than that in IR group and M3 group （P〈0.05）. Conclusion The best memory phase of the cardioprotective effect of IsoP is about 20min in isolated rat heart.
