摘　　要：椎管内应用阿片类药物因为其诸多优点近年来在临床上应用广泛。新的药物和给药方法层出不穷。然而，对于阿片类药物在椎管内的作用机制、分布、头向移动、最佳剂量及长期给药的优缺点均尚存争议。Intravertebral administration of opioids has become more and more popular in clinical practice for its advantages. New drugs and new administration ways are coming out one after other. However, there are still controversies on the mechanism with that opioid acts on spinal cord, its distribution within epidural or intrathecal space, rostral spread and optimal dose. Liposolubility is the extremely important factor to affect the efficacy of opioids which mostly depends on its biologic nature. However, the effect of opioids may sometimes changes in case of combined administration with local anesthetics, or with the different administration way.
