摘  要:目的探讨呼出气一氧化氮(eNO)在支气管哮喘(AS)儿童中的应用,比较eNO和血清嗜酸粒细胞阳离子蛋白(ECP)在AS、AS合并过敏性鼻炎(AR)、慢性咳嗽变异性哮喘(CVA)的不同,寻找适用于儿童AS简便易行的气道炎症监测指标方法。方法设定50ml/s呼出气流速,采用电化学法对51例5~14岁患有AS、AS/AR、CVA的患儿,及30例5~14岁正常对照组儿童进行eNO测定,同时测定ECP及第1秒用力呼气容积占预计值百分比(FEV1pred%)。结果AS、AS/AR、CVA三组间eNO、ECP水平均高于对照组(P<0.01);AS/AR组eNO、ECP水平均高于AS、CVA组(P值均<0.05),而AS、CVA组间则无明显差异(P>0.05);AS组eNO与ECP间存在显著相关性(r=0.64,P<0.05),但与FEV1间则无明显相关性(r=0.144,P>0.05)。结论eNO水平在AS/AR组增高,提示eNO在过敏性体质中高表达。eNO浓度与血清ECP呈正相关,提示eNO可以反映AS患者气道嗜酸性炎症水平。Objective To discuss the application of exhaled nitric oxide (eNO) on children with bronchial asthma(AS) ,to compare with the difference of eNO and eosinophile cationic protein(ECP) among AS, AS/allergic rhinitis(AS/AR), and chronic cough variant asthma(CVA). To search for easy indicators for monitoring airway inflammation that is apply to children with asthma. Methods To measure eNO from 51 cases of children who are suffering from AS,AS/AR,CVA,and 30 cases of children in normal control group by using light-emitting electrochemical photometry,at the same time,to determine (ECP) and FEV1 pred%. Results Compare to the normal control group,eNO concentration in children with AS is significantly higher ( P 〈0.05). There is significant correlation between eNO concentrations and ECP in children with AS ( r =0. 394, P 〈0.05), there is no significant correlation between eNO concentrations and FEV1 pred% (r = 0. 144, P 〉 0.05) children with AS, there is no significant difference in eNO concentration among the children who are suffering from AS,AS/AR,CVA( P 〉0.05). Conclusions eNO levels in AS/AR group increased,suggest that eNO in allergic constitution is highly expressed, eNO concentration was positively correlated with serum ECP,suggest that eNO may reflect AS airway eosinophilic inflammation levels.
