摘 要:目的探讨参照棘突与椎弓根在影像学上的角度关系,确定术中AF螺钉的方向。方法术前对23例胸腰椎骨折X线侧位片及CT片进行研读,测量椎弓根与棘突的角度关系,指导术中椎弓根螺钉的植入方向。结果术后X线片显示:23例患者椎弓根螺钉位置良好,骨折复位满意,椎体高度恢复80%以上,Cobb角由术前12°-41°矫正至0°-5°。结论利用棘突与椎弓根在影像上的角度关系确定椎弓根螺钉方向,是术中椎弓根螺钉定向的有效方法。Objective To explore the angle relation between spinous process and the pedicle of vertebral arch in image,and to decide the direction of screws of AF system.Methods Before the operation,the angles between the spinous process and the pedicle of vertebral arch were detected,and used to decide the direction of screws in operation.23 cases of thoracolumbar vertebra fracture were treated with AF system in this way.Results The X-ray photographs after operation showed that the localizations of screws of 23 cases were all correct,the vertebral height was recovered satisfactorily over 80%,and Cobb angles were reduced from pre-operative 12°41° to post-operative 0°5°.Conclusions To decide the direction of the screw of AF system with the angle relation between spinous process and pedicle of vertebral is a valid idea.
