硝酸甘油联合艾司洛尔用于术后降压的效果观察

摘　　要：目的比较硝酸甘油联合艾司洛尔与单纯硝酸甘油术后降压的效果。方法选择肺癌全麻术后合并高血压的患者42例随机分为两组,每组21例。A组予硝酸甘油复合艾司洛尔降压,B组单纯用硝酸甘油降压,观察并比较两组降压时血液动力学变化。结果两种方法均能有效地降压,但艾司洛尔能明显加快硝酸甘油的起效速率,减少硝酸甘油用量,并能抑制单纯用硝酸甘油降压时出现的反射性心率增快,心肌耗氧指数下降,与单纯硝酸甘油组比较差异有显著性。结论硝酸甘油复合艾司洛尔降压有协同作用,无明显反跳现象,是一种安全有效的术后降压方法。Objective To compare the anti-hypertensive effects between nitroglycerin combined with esmolol and nitroglycerin alone. Methods 42 patients with hypertension after lung cancer resection under general anesthesia were randomly divided into 2 groups,21 patients in each. Group A was received nitroglycerin combined with esmolol, Group B was treated with nitroglycerin alone. Observed and compared the hemodynamic changes between two groups. Results Both methods were effective in decreasing high blood pressure. However, esmolol significantly speeded up effect and reduced amount of nitroglycerin, inhibited reflexing faster heart rate by nitroglycerin only, and decreased myocardial oxygen consumption index, compared with nitroglycerin alone. Conclusion Nitroglycerin combined with esmolol has synergistic effects, no obvious rebound phenomenon, which is a safe and effective approach for treating postoperative hypertension.
