循环血HSP70在急性创伤患者诊治中的作用

摘　　要：目的研究急性创伤患者循环血热休克蛋白(HSP)70浓度的变化,探讨循环血HSP70在创伤患者诊断、治疗中的作用。方法将57例急性创伤患者按照ISS评分标准分成轻、中、重三组;收集患者伤后1～6、24～36、60～90 h等不同时间段的血液标本,检测血清HSP70浓度。统计分析各组患者血清HSP70浓度变化、颅脑损伤与其他损伤的差别以及合并感染与未合并感染患者之间的差别。结果创伤各组三个时间段血清HSP70浓度均显著高于对照组;重伤组三个时间段血清HSP70浓度显著高于轻、中度伤组;颅脑损伤患者前两个时间段血清HSP70浓度显著高于其他创伤组;感染患者组从第二时间段开始血清HSP70浓度显著高于非感染组。结论急性创伤患者循环血HSP70浓度可以帮助判断伤情、鉴别诊断颅脑损伤与非颅脑损伤,持续升高可预示感染。Objective To study the changes of circular blood HSP70 in patients with acute injury, and to discuss the effect of circular blood HSP70 on patient＇ s diagnosis and therapy. Methods Fifty- seven patients with acute injure were divided into mild group, medium group, and severe group according to the ISS. Blood specimens of injured patients were collected at different stages after injury 1-6 hours, 24-36 hours and 60-90 hours, the concentration of serum HSP70 was tested, and then, statistical analyses were performed with SSPS11.5. Results The concentrations of serum HSP70 of three injured groups were significantly higher than that of control group, and the concentrations of serum HSP70 at the first, second and third stages were significantly higher in the severe group than the other two injured groups. The concentration of serum HSP70 of TBI patients was significantly higher than that of non- TBI injured group at the first and second stages. The concentration of serum HSP70 was significantly higher in inflection group than in non- inflection group from the second stage. Conclusions Circular blood Hsp70 can help estimating the degree of injury and distinguishing between TBI and non - TBI, its persistent increase is the symbol of infection.
