摘 要:目的观察盐酸戊乙奎醚对失血性休克手术病人微循环的影响。方法选择50例失血性休克需急诊手术的患者,随机分成盐酸戊乙奎醚组(Ⅰ组,n=25)和对照组(Ⅱ组,n=25),I组患者麻醉诱导前予盐酸戊乙奎醚0.5mg静脉注射,同时3mg肌肉注射;Ⅱ组除未用盐酸戊乙奎醚外,余处理同Ⅰ组。观察两组患者指端的血氧饱和度(SPO2)及动脉血氧饱和度(SaO2),麻醉诱导1h后测量动脉血乳酸(BL)含量,并记录此时的心率(HR)、平均动脉压(MAP)以及整个手术中尿量。结果Ⅰ组BL较Ⅱ组显著降低(P<0.05);Ⅰ组SPO2较Ⅱ组显著提高(P<0.05);Ⅰ组术中尿量较Ⅱ组呈显著增多(P<0.05)。结论盐酸戊乙奎醚能明显改善失血性休克病人微循环的功能。Objective To observe the effect of penehyclidine on microcirculation in patients with hemorrhagic shock. Methods 50 received emergency patients with hemorrhagic shock were randomly divided into 2 groups : penehyclidine group ( group Ⅰ , n = 25) and control group ( group Ⅱ , n = 25 ). Before anesthesia induction, 0. 5mg and 3mg penehyclidine were respectively intravenous and intramuscular injection in the patients of penehyclidine group, however, the patients of control group were not injected. Pulse oxygen saturation ( SpO2 ) and arterial oxygen saturation (SaO2 ) were measured in two group, heart rate (HR), mean arterial pressure (MAP) and blood lactate(BL) were monitored after 1 hour of anesthesia induction, and urinary output during operation were recorded. Results Compared with group Ⅱ, blood lactate were decreased significantly in the patients of group Ⅰ( P 〈 0.05 ), and SpO2 and urinary output were increased significantly in the patients of group Ⅰ ( P 〈 0.05 ). Conclusion Penehyclidine can improve the function of microcirculation in patients with hemorrhagic shock.
