卡贝缩宫素减少腹腔镜子宫肌瘤剔除术术中出血

摘　　要：目的评价卡贝缩宫素在腹腔镜下子宫肌瘤剔除术中促进子宫收缩、减少出血的效果。方法58例行腹腔镜下子宫肌瘤剔除术的病人分成2组:卡贝缩宫素组30例,缩宫素组28例,比较2种药物对术中出血量、手术时间及术后血红蛋白下降的影响。结果58例手术均获成功,无术中或术后并发症。卡贝缩宫素组术中出血量明显少于缩宫素组[(111±s35)mL vs.(160±49)mL,P<0.01],术后血红蛋白下降也明显低于后者[(13±8)g·L^(-1) vs.(18±10)g·L^(-1),P<0.05];2组的手术时间没有显著差异[(123±33)min vs.(128±30)min,P>0.05]。结论卡贝缩宫素应用于腹腔镜下子宫肌瘤剔除术可有效促进子宫收缩,减少术中出血量。AIM To evaluate the effectiveness of carbetocin in reducing hemorrhage during laparoscopic myomectomy. METHODS Fifty-eight patients underwent laparoscopic myomcctomy with carbetocin （n = 30） or oxytocin （n = 28） during operation were analysed. The blood loss, reduction of haemoglobin, and the operation time were compared. RESULTS All operations were performed successfully with no operation-related complications occurred in these cases. The mean blood loss in operation of the carbetocin group were significantly less than that of the oxytocin group （（111± s 35） mL vs. （160 ± 49） mL, P 〈 0.01）, the mean reduce of haemoglobin of the carbetocin group were also less than that of the oxytocin group （（13 ± 8） g.L^-1 vs. （18 ± 10） g.L^-1, P 〈 0.05）, and there was no significant difference in operation time between the two groups （（123 ± 33） min vs. （128 ± 30） min, P〉 0.05）. CONCLUSION Carbetocin may stimulate contraction of the uterus and reduce blood loss during laparoscopic hysterectomy.
