摘  要:目的对比非甾体类药物(NSAIDs)、透明质酸钠(HA)、骨髓间质干细胞(BMSCs)三种方法治疗膝骨关节炎各个时期的疗效。方法对60例关节镜下确诊为膝骨关节炎的病人随机分3组,分别用:(1)术后用NSAIDs治疗2~4周;(2)术后注射HA1~2疗程;(3)术后取髂脊骨髓进行分离培养出BMSCs再进行回植到膝关节中等3种方法治疗。分别在术后3个月、1年、3年的时间进行随访、并用WOMAC指数评分法对膝关节功能进行评价,两两对比组间的治疗效果。结果3个月随访显示3组在治疗结果方面差异无统计学意义(P>0.05);1年随访显示HA组和BMSCs组与NSAIDs组相比治疗结果差异有统计学显著意义(P<0.05);3年随访显示3组间治疗效果都有统计学显著差异。结论3种方法对骨关节炎的早期疗效都较好,但BMSCs的长期效果要优于其他2种方法。Objective To compare the efficacy of non-steroidal drugs ( NSAIDs ), hyaluronic acid ( HA), bone marrow mesenchymal stem cells ( BMSCs ) for knee osteoarthritis of various periods. Methods Sixty patients with knee osteoarthritis confirmed by arthroscopic operation were randomly divided into 3 groups : ( 1 ) NSAIDs group was treated with NSAIDs for 2 ~ 4 weeks after operation ; (2) HA group was treated by injection of HA 1 ~ 2 courses after operation; (3) BMSCs group was treated by intra-artiealar replantation of BMSCs obtained by means of isolation and culture of iliac crest bone marrow. The knee joint function were evaluated by WOMAC index scores ,3 months, 1 year, 3 years after surgery. Pairwise comparison between the three groups were performed. Results 3 months follow- up showed the treatment results of three groups was not statistically difference ; 1 year follow-up showed in comparison of HA group and the BMSCs group with the NSAIDs the treatment results were statistically difference ;3-year follow-up showed three groups were statistically difference between the treatment effect. Conclusion The early efficacy of three kinds of methods for osteoarthritis are good, but the long-term effects of BMSCs group is superior to the other 2 methods.
