摘 要:〔目的〕探讨HIV感染者/艾滋病病人的CD4+T淋巴细胞数与淋巴细胞总数(tota1 lymphocyte count,TLC)的相关性,了解用TLC预测CD4+T淋巴细胞数的可行性。〔方法〕采用SPSS14.0统计软件分析136例HIV感染者/艾滋病病人,共计144对TLC与CD4+T淋巴细胞数之间的相关性。根据敏感度、特异度、功效率、阳性预测值和阴性预测值,预测CD4<200个/μl和CD4<350个/μl时的TLC最佳临界值。〔结果〕CD4+T淋巴细胞与淋巴细胞总数间呈显著正相关(r=0.6281,P<0.01)。用TLC<1300个/μl预测CD4<200个/μl敏感度为42.42%,特异性为91.74%,功效率为81.17%,阳性预测值为58.33%,阴性预测值为85.38%;用TLC<2 200个/μl预测CD4<350个/μl敏感度为72.50%,特异性为76.19%,功效率为74.39%,阳性预测值为74.36%,阴性预测值为74.42%。回归方程:CD4+T淋巴细胞数(个/μl)=16.708+0.158×总淋巴细胞数(个/μl)。〔结论〕在条件有限的地区,可以用淋巴细胞总数预测CD4+T淋巴细胞数,监测HIV感染者疾病的进展情况,粗略判断抗病毒治疗的疗效。Objective To study the correlation between total lymphocyte counts(TLC) and CD4+T lymphocyte counts in HIV/AIDS patients,and to evaluate the feasibility of TLC in predicting CD4+T lymphocyte counts.Method Correlation between TLC and CD4+T lymphocyte counts were analyzed by SPSS14.0 Software in 144 samples collected from 136 HIV/AIDS patients.The sensitivity,specificity,efficiency,positive predictive value(PPV) and negative predictive value(NPV)of various TLC critical value were computed for CD4T 〈 200 for CD4 T 〈 200 cells/μl and CD4 〈 350 cell/μl. Result A significant positive linear correlation was noted between TLC and CD4+ T lymphocyte counts(r = 0.6281 ,P〈 0.01).TLC 〈 1 300 cells/μl had 42.42% sensitivity,91.74% specificity,81.17% efficiency,58.33% PPV and 85.38% NPV for predicting CD4〈 200 cells/μl. TLC 〈 2200 cells/μl had 72.50% sensitivity,76.19% specificity,74.39% efficiency, 74.36%PPV and 74.42% NPV for predicting CD4〈350 cells/μl.Linear regression eguation:CD4+ T lymphocyte counts (cells/μl)=16.708+0.158xTLC (cells/μl). Conclusion TLC may be used to predict the CD4~ T lymphocyte count and monitor the disease progression and evaluate the curative effect of highly antiretroviral therapy in HIV-infected patients in resource-limited area.
