腰麻-硬膜外联合麻醉应用于70岁以上高龄患者的安全性观察

摘　　要：【目的】探讨硬膜外-腰麻联合麻醉（csEA）用于高龄患者下腹部及下肢手术的临床可行性及安全性。【方法】选择100例在硬膜外-腰麻联合麻醉下行下腹部及下肢手术年龄大于70岁的患者，ASAⅠ～Ⅲ级，评定阻滞平面、收缩压、舒张压、心率及脉搏血氧饱和度的变化。【结果】硬膜外-腰麻联合麻醉起效时间短，止痛效果完善，麻醉过程平稳，用药量少。【结论】硬膜外-腰麻联合麻醉用于高龄患者下腹部及下肢手术，只要麻醉管理得当，对高龄患者不会产生不良影响，是一种安全可靠的麻醉方法。[Objective]To explore the feasibility and safety of combined spinal and epidural anesthesia （CSEA） in elderly patients （aged 70 years or over） undergoing lower abdominal or limb operation. [Methods] A total of 100 cases （aged 70 years or over） with ASA Ⅰ-Ⅲ undergoing lower abdominal or limb operation with CSEA were observed. Anesthesia efficacy, hemodynamic influence and side effect were also observed. [Results]CSEA had significant effects of fast anesthesia, good analgesia, fine muscle relaxation, little adverse effects and less drug consumption. [Conclusion] CSEA is safe and effective for lower abdominal or limb operation in patients aged 70 years or over.
