摘 要:目的研究林州市食管癌(Esophageal carcinoma,EC)主要危险因素,并探寻存在相互矛盾的可能原因。方法收集林州市主要医院经病理学确诊的连续性住院病例作为病例组;选择同期同院、年龄和居住年限相近的多科室多病种非肿瘤患者为对照组,进行病例对照研究。结果EC家族史(OR:2.59,P=0.001)、食用酸菜(OR:2.02,P=0.015)和少食动物性食物(OR:2.17,P=0.004)为主要危险因素;男性每日吸烟30支及以上者与EC明显有关(OR:2.19,P=0.042);食用酸菜(年限、停食年限)和吸烟(每日吸烟量)均与EC的发生存在明显剂量效应关系。结论家族史为林州市食管癌的稳定的危险因素,食用酸菜为重要危险因素,男性过量吸烟也已成为重要的危险因素。Objective Aimed to verify the major risk factors and to resolve or explain the contradictions in Linzhou City. Methods A hospital based case control study was conducted. All the patients who had been diagnosed pathologically at the main hospitals in Linzhou City were recruited as the case group whereas non - cancer patients with various diseases in various departments at the same time and hospital, with similar age and length of stay, were used as the control group. Results Multivariate analysis indicated that family history of esophageal carcinoma (OR: 2.59, P =0. 001 ) was an important risk factor and eating pickled vegetables (OR: 2.02, P = 0. 015 ) and less consumption of animal foods ( OR: 2.17, P = 0. 004) were also significant factors. Males smoking 30 or more cigarettes a day had a significant risk of developing esophageal carcinoma ( OR = 2.19, P = 0. 042). There was a significant dose -effect relationship between the incidence of esophageal carcinoma and eating pickled vegetables (diet years and abstinence years) and smoking ( daily smoking amount). Conclusions tn Linzhou City, family histmy of esophageal carcinoma is a major risk factor for developing esophageal carcinoma. The effect of overeating pickled vegetables is still the important risk factor. Heavy smoking among males is also an important risk factor.
