摘  要:目的:探讨腔内微创技术在治疗膝下动脉硬化性闭塞症(ASO)中的价值。方法:采用患肢顺行穿刺加长球囊,对46例膝下段ASO患者51条肢体行经皮腔内血管成形术(PTA)。结果:患肢PTA技术成功率88.2%(45/51)。术后随访6个月~2年,94.1%(48/51)患肢的发凉、麻木、疼痛等症状有不同程度缓解;88 2%(45/51)患肢温度增高、行走距离加长;5条患肢足踝部溃疡愈合;2条患肢足小趾坏死自行脱落,创面愈合。患肢踝/肱指数(ABI)由术前的0.58±0.13升至术后的0 89±0.15,最近一次随访测量值为0.85±0.14。结论:顺行穿刺加长球囊行PTA技术治疗膝下段ASO近期疗效确切。Objective:To evaluate the effects of enclovascutar non/nvasive technology for the treatment of atherosclerotic occlusion of infrapopliteal artery. Methods:Application antegrade puncture and long balloon in femoral artery in 46 atheroscterotic occlusive disease infrapopliteal artery with 51 limbs were used and percutaneous transluminal angioplasty was performed, antico- agulant and vasodilator medicine, such as low molecule heparin, argatroban and alprostadil were given after operation. Results:The technical success was obtained in 45 limbs, the technical suc- cess rate was 88.23% in 51 suffered limbs. During a follow-up period of 6 to 24 months, pain relieved in 48 limbs, skin temperature increased and walk distance prolonged in 45 limbs. Five limbs with preoperative foot ulcer were healed. Two limbs with foot gangrene were self-failed off and healed of surface of the wound. The ABI was increased from 0.58±0.13 in preoperation to 0,89 ±0.15 in postoperation,and the last examination was 0.85±0.14(P〈0.05).Conclusion:Antegrade puncture and long balloon PTA is a safe and effective treatment for patients with atherosclerotic occlusive disease of infrapopliteal artery,it has exact neer-term therapentic efficacy.
