摘 要:目的评价无冠状动脉压力监测下冠状动脉造影术临床应用的安全性、有效性和优势。方法2006年4月~2009年4月在赣州市立医院心血管内科和心脏外科住院患者836例,行选择性冠状动脉造影,随机分为无冠状动脉压力监测下行冠状动脉造影术组418例(未用三联三通极,改用普通小三通,简称无压力监测组)和常规冠状动脉造影术组418例(用三联三通监测冠状动脉压力,简称压力监测组),比较两组手术操作时间、X线照射时间、手术成功率、冠状动脉并发症(冠状动脉痉挛,导管嵌顿、阻塞冠脉开口)、恶性心律失常、材料费用。结果两组患者X线照射时间分别为(3.67±2.45)min和(3.71±2.61)min(P>0.05);手术操作时间分别为(14.67±2.95)min和(21.36±8.47)min(P<0.05);冠状动脉并发症分别为5例和4例(P>0.05),两组均无恶性心律失常发生,材料费分别为(1063.42±162.25)元和(1337.08±194.31)元(P<0.05)。结论无冠状动脉压力监测下行冠状动脉造影术和常规冠状动脉压力监测下行冠状动脉造影术一样安全、有效和可行,且比压力监测组节省费用,缩短手术操作时间和减少医生X线照射时间。Objective It is to evaluate safety, efficiency and advantage of under no coronary artery pressure monitoring of coronary angiography clinical application. Methods In April 2006- 2009 April in GanZhou Municipal Hospital cardiovascular physician and cardiac surgery patients, 836 cases in all.Hospitalized patients with the selective coronary angiography, randomized without coronary artery pressure monitoring descending coronary angiography 418 patients group(Unused sanlian tee, use common small tee, referred to as pressure monitoring group). Conventional coronary angiography 418 patients group(sanlian tee monitoring coronary artery pressure, referred to as pressure monitoring group). Two groups of operation time and X-ray irradiation time, surgical success shot, coronary artery complications (coronary artery spasm, and coronary artery catheter obstruction embedded mouth), malignant arrhythmia, material fee. Results Two groups of patients, X-ray irradiation time respectiveIy(3.674-245)min and (3.71 4- 2.61)min(P〉0.05), Surgical operation time respectively(14.67+2.95)min and (21.36+8.47)min(P〈0.05);coronary artery complications in 5 cases and respectively, 4 cases(P〉0.05); two groups are no malignant arrhythmia, material fee respectively (1063.42e 162.25) and (1337.084-194.3 l) yuan (P〈 0.05). Conclusion Without coronary artery pressure monitoring descending coronary angiography and conventional coronarY artery pressure monitoring descending coronary angiography as safe, effective and feasible, and pressure monitoring group than saving cost, shorten the operation time and reduce the doctor X-ray irradiation time.
